<p>Title: Social Worker/ Counselor- (LMSW/LPCA)</p> <p>Reports to: Director of Outpatient Services</p> <p>Department: Outpatient Clinic</p> <p>Classification: Exempt: Non-Union</p> <p>Date Created/Revised: 10/12, 03/14, 03/15, 04/17, 04/18, 6/22, 4/23, 12/25</p> <p>Organization Overview:</p> <p>Sound Community Services, Inc. is a private, not-for-profit organization dedicated to educating, assisting, and supporting individuals with persistent mental illness and substance use disorders. We work tirelessly to create a culture and environment in which recovery and wellness are possible.</p> <p>Position Overview:</p> <p>Provide intake assessments, group therapy, and coordinate referrals to appropriate services as recommended. Provide individual and group therapy in a community-based outpatient clinic. Using CareLogic software, the Clinician will document all clinical services provided in accordance with DMHAS and SCSI expectations. The Clinician is expected to manage the caseload assigned, including all documentation, in a timely manner, and to attend team meetings and clinical supervisions.</p> <p>Essential Function:</p> <ul> <li>Provides clinical services to persons enrolled in the outpatient program. </li><li>Documents outpatient services rendered through the completion of progress notes that include description of the nature of the service, the persons response to the therapeutic intervention and its relation to the goals of treatments. </li><li>Provides individual, group and family therapy to an assigned case load. </li><li>Completes treatment plan reviews that include updates on client progress at least every ninety (90) days and discharge summaries whenever a case is closed. </li><li>Documents all client contacts in the client's record. </li><li>Completes all documentation i.e. referrals, treatment plans and reassessments and progress notes in accordance with the agency's Quality Assurance standards and in a timely manner. </li><li>Provides appropriate linkage with community resources, e.g., meal centers social rehabilitation, case management and residential services. </li><li>Demonstrates comprehensive understanding of all agency programs and program criteria. </li><li>Documentation is legible and relates directly to recovery plan goals. </li><li>Master electronic health record functionality. </li><li>Manages case load, regular reviews of CareLogic System, performing utilization review independently. </li><li>Participates in individual and group clinical supervision, multi-disciplinary team meetings, agency committees and program safety drills. </li><li>Participates in community awareness presentations as a representative of SCSI. </li><li>Adheres to all agency policies and procedures, including but not limited to confidentiality and the code of ethics. </li><li>Attends and participates in training, both to maintain State of CT licensure, based on the needs of the agency and populations served. </li><li>Always maintains confidentiality. </li><li>Meets productivity requirements, as outlined by the agency. </li><li>Will attend adequate clinical supervision by LCSW to achieve successful completion of licensure testing in two (2) years from start date- resulting in LCSW/LPC licensure. </li><li>Performs all other duties as assigned. </li></ul> <p>Physical Requirements:</p> <p>While performing the duties of this job, the employee is regularly required to sit, use hands to finger, handle or feel objects, tools, or controls, and climb stairs. The employee frequently is required to walk, talk and/or hear; Speaking and hearing ability sufficient to communicate effectively by phone or in person at normal volumes; Vision adequate to read correspondence, computer screen, forms, etc. The employee is occasionally required to stand and climb or balance. The employee must occasionally lift and/or move up to 75lbs. Specific vision abilities required by this position include the ability to adjust focus.</p> <p>The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.</p> <p>Qualifications:</p> <p>Master's degree in behavioral health science; LMSW/LPCA license.
