<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.</p> <p>VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU</p> <p>VA Careers - Licensed Clinical Social Worker: https://youtube.com/embed/U_xC25QsN0w</p> <p>Total Rewards of a Allied Health Professional</p> <p>The Social Worker Pact Aligned Care Team position is in Social Work services at the Bedford VA Medical Systems located at the Bedford, Massachusetts location. This position is full-time at 40 hours per week. The Patient Aligned Care Team (PACT) Social Worker serves as a member of the PACT team providing a variety of clinical interventions to Veterans in the primary care setting. PACT social workers assist with care coordination and access to care for Veterans. Duties include but are not limited to:</p> <ul> <li>Conducts comprehensive psychosocial assessments, including need and social risk factors, to determine needs of Veterans and family members; </li><li>Participates actively in the treatment planning process as part of interprofessional team; </li><li>Participates in discharge planning with other professions and assists with ensuring discharge plans are executed timely and appropriately; </li><li>Provides individual, family and group consulting as appropriate; </li><li>Provides education to Veterans, their families, caregivers and loves ones community resources, advance directives and VA Benefits; </li><li>Acts as a consultant to other team and staff members; </li><li>Provides consultation and education to other professions on social work practice and VA policy and procedure; </li><li>Acts as a liaison between VA and community resources, to include outreach activities as needed; </li><li>Provides comprehensive casa management services to Veterans, including those with high risk factors; </li><li>Provides a full range of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate; </li><li>Provides Whole Health, wellness and prevention education to Veterans, Families, caregivers and loved ones in individual or group settings; </li><li>Provides crisis intervention services consisting of assessment, evaluation and referral; </li><li>Coordinates treatment across departments; </li><li>Supports Veterans with coping and dealing with the loss and grief experiences in disability, terminal illness and death; </li><li>Documents social work interventions and activities in electronic health record; </li><li>Ensures appropriate hand off when transferring of patient care; </li><li>Adheres to all documentation standards and timelines; </li><li>Participates in committee assignments, meeting and in projects as assigned; </li><li>Maintains data and statistical compilations to comply with The Joint Commission, other accreditation bodies, VA and medical center policy and procedures; and </li><li>Other duties as assigned. </li></ul> <p>Work Schedule: Monday-Friday; 8:00am-4:30pm</p> <p>Recruitment Incentive (Sign-on Bonus): Not Authorized</p> <p>Permanent Change of Station (Relocation Assistance): Not Authorized</p> <p>EDRP Authorized: Former EDRP participants ineligible to apply for incentive.
