<p>02200 Social Worker</p> <p>Assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.</p> <p>Reports to: Supervisor of Social work</p> <p>FLSA: Non-Exempt</p> <p>Education: Bachelors required; Masters preferred</p> <p>Licenses: Board Certified Social Worker, or related field (i.e. counseling) in the state of Kentucky (CSW, LPCA, LCSW) within 6 months of hire</p> <p>Certifications: Handle With Care within 30 days of hire.</p> <p>Required Skills</p> <p>Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.</p> <p>Essential Functions</p> <p>Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.</p> <p>Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.</p> <p>Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.</p> <p>Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.</p> <p>Supervises and/or trains new staff, students, and interns.</p> <p>Provides individual, family and group therapy.</p> <p>Non-Essential Functions</p>
