<p>Mercy Community Healthcare has an opening for a Patient Navigator in Franklin, TN. We are looking for enthusiastic people with a passion for making a difference in the lives of those we serve in our community. This position provides case management services to patients and their families, addressing medical needs as well as social determinants of health.</p> <p>Mercy's Mission: The mission of Mercy Community Healthcare is to reflect the love and compassion of Jesus Christ by providing excellent healthcare to ALL and support to their families. We provide care to the uninsured, underinsured and insured through pediatric and adult primary care, mental and behavioral health services, and care coordination.</p> <p>Essential Responsibilities</p> <p>Serve as a contact, advocate, and informational resource for family, community, and other social or medical agencies.</p> <p>Schedule subspecialist referrals. Document all relevant information in the EMR, including date/time/location of the appointments.</p> <p>Advocate for needed services on behalf of patients, such as medication cost assistance or transportation barriers. Learn continuously to best address these concerns.</p> <p>Meet face-to-face with clients and families for paperwork and discussion of referrals.</p> <p>Maintain updated resource information regarding community supports and options for patients.</p> <p>Attend outreach functions and gather helpful contacts to share with staff.</p> <p>Be a resource hub for medical and social service needs of our patients and community.</p> <p>Become CAC insurance trained.</p> <p>Bilingual in English and Spanish preferred.</p> <p>Qualifications</p> <p>Education: Bachelor's degree in healthcare or social service field preferred. High school diploma required. Willingness to become certified application counselor preferred.</p> <p>Experience: At least one year of office experience or healthcare preferred but not required. Phone-based experience and/or work with an EMR is ideal.</p> <p>Functional Competencies: Interpersonal skills, Teamwork, Oral and Written Communication, MS Office and strong typing skills, Ability to Multitask, Cultural Competency, Problem-Solving, Organization, Patience.</p>
