<p>HUD-VASH is an intensive case management program for homeless Veterans aimed at providing psychosocial services to Veterans while residing in stable housing through HUD's Section 8 voucher system. The incumbent in this position functions as the Case Manager for Tomah VAMC's Housing and Urban Development / VA Supportive Housing (HUD-VASH).</p> <p>Total Rewards of a Allied Health Professional</p> <p>Major duties include but not limited to:</p> <p>Clinical</p> <ul> <li>Demonstrate a high level of skill and expertise to establish and maintain an effective therapeutic relationship with Veterans and/or their families, caregivers, or survivors. </li><li>Demonstrate a high level of skill and knowledge of Veteran treatment and disposition in accordance with criteria for care planning across the Veteran care continuum and in a variety of clinical settings and levels of care. </li><li>Demonstrate an ability to complete thorough assessments to determine the psychosocial functioning and needs of Veterans and/or their families, caregivers, or survivors. </li><li>Actively participates as a member of the interdisciplinary treatment team in the development and implementation of treatment goals and interventions as well as collaborate with Veterans and/or their families, caregivers, or survivors. </li><li>Serves as a representative of the VA and participates in/coordinates educational outreach of VA resources and services. </li></ul> <p>Administrative</p> <ul> <li>Attend all appropriate staff meetings. </li><li>Collaborate in performance improvement processes and comply with performance </li></ul> <p>measures as required by the VA for their specific population and/or program.</p> <ul> <li>Participate in activities that obtain feedback from Veterans and work to enhance </li></ul> <p>services as needed.</p> <ul> <li>Participate in the monthly Peer Review process. </li><li>Perform other duties as assigned. </li></ul> <p>Work Schedule: Monday-Friday; 8:00am to 4:30pm</p> <p>Permanent Change of Station (Relocation Assistance): Not Authorized</p> <p>Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).</p> <p>Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
