This position will directly report to the VP of Palliative Care and is responsible for assisting in the development, implementation, and evaluation of operational policies and procedure, partner workflows, and bi-directional communication surrounding service delivery and contract execution related to Enhanced Palliative Partnerships. Responsible for facilitating communication and coordination between operational leaders in palliative and hospice and the partners, acting as a bridge to ensure smooth and effective care and communication.
