There is something for everyone in this culturally diverse city.</p> <p>Job Summary:</p> <p>Provides psychosocial assessments, diagnosis, and treatment, as well as discharge planning to and consultation about patients and families to assist them and the health care team in coping with patient's hospitalization, illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders. Functions in a supervisory role as a team leader.</p> <p>Job Requirements:</p> <p>Education and Work Experience:</p> <ul> <li>Master's Degree in social work: Required </li><li>One year experience in an acute or long term medical setting: Required </li><li>Two years' experience in an acute or long term medical setting: Preferred </li></ul> <p>Licenses/Certifications:</p> <ul> <li>Licensed Clinical Social Worker (LCSW) in state of practice: Required </li></ul> <p>Essential Functions:</p> <ul> <li>Collaborates and facilitates patient discharge plans for those requiring post Hospice or post inpatient care, ensuring that the required paperwork is completed. </li><li>Assists patient/family/caregiver in determining appropriate level of Hospice care. </li><li>Initiates a plan of care and appropriate patient teaching. Reassesses plan of care according to the physical and emotional needs of patient. </li><li>Develops social work Plan of Care (POC) based on patient assessment including, psychosocial factors that would impede the POC, clinical knowledge of appropriate theories of stages of human development, the impact of terminal illness, mental status and family dynamics. Documents relevant data accurately and in compliance with laws and regulations. </li><li>Actively participates in training and/or cross-training of duties. </li><li>Performs other job-related duties as assigned. </li></ul> <p>Organizational Requirements:</p> <p>Adventist Health is committed to the safety and wellbeing of our associates and patients.
