Work for the best and join the Heart of the CarDon family of care.</p> <p>What you'll do</p> <p>The Regional Assessment Coordinator (RN) at CarDon Corp plays a crucial role in ensuring the highest standards of patient care and compliance across multiple facilities. This position is responsible for coordinating and overseeing the assessment processes for residents, ensuring that their needs are accurately identified and met. The coordinator will work closely with nursing staff, management, and interdisciplinary teams to develop and implement care plans that enhance the quality of life for residents. Additionally, the role involves training and mentoring staff on assessment protocols and best practices. Ultimately, the Regional Assessment Coordinator will contribute to the overall mission of CarDon Corp by promoting a culture of excellence in care delivery.</p> <p>Job Responsibilities</p> <ul> <li>Conduct comprehensive assessments of residents to determine their healthcare needs and preferences. </li><li>Collaborate with nursing staff and interdisciplinary teams to develop individualized care plans. </li><li>Monitor and evaluate the effectiveness of care plans, making adjustments as necessary. </li><li>Provide training and support to staff on assessment tools and documentation requirements. </li><li>Ensure compliance with state and federal regulations regarding resident assessments and care planning. </li></ul> <p>Our ideal candidate</p> <p>Minimum Qualifications</p> <ul> <li>Current Registered Nurse (RN) license in the state of practice. </li><li>Bachelor's degree in Nursing or a related field. </li><li>Minimum of 2 years of clinical experience in a healthcare setting. </li></ul> <p>Preferred Qualifications</p> <ul> <li>Experience in long-term care or geriatric nursing. </li><li>Previous experience in a coordinator or supervisory role. </li><li>Certification in gerontological nursing (CGN) or related specialty. </li></ul> <p>Skills</p> <p>The required skills for this position include strong clinical assessment abilities, which are essential for accurately identifying residents' needs. Effective communication skills are vital for collaborating with various teams and ensuring that care plans are understood and implemented. Organizational skills are necessary to manage multiple assessments and documentation efficiently. Preferred skills, such as experience in geriatric nursing, enhance the coordinator's ability to relate to residents and understand their unique challenges.
