<p>The VA Northeast Ohio Healthcare System is recruiting for two Social Workers in the Mental Health Residential Rehabilitation Treatment Program (MHRRTP). The incumbent provides case management services and residential treatment services for veterans in recovery from substances use disorder, mental illnesses, and/or homelessness. The primary work setting is Domiciliary located at Louis Stokes Cleveland VA Medical Center.</p> <p>The Social Worker (MHRRTP) duties include, but are not limited to:</p> <ul> <li>Completes clinical tasks including Suicide risk screening and safety plans if needed, Advance Directives, Psycho-social assessments, and treatment planning in collaboration with veterans/families and the team. </li><li>Offers psychosocial interventions, including case management and supportive counseling, educational groups, and utilize the milieu to enhance the ability of the individual and group to reach goals of wellness and recovery. </li><li>Provides case management and group treatment to the veteran to assist them with accessing resources needed to promote wellness/recovery and independent living. </li><li>Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agencies programs. </li><li>Establishes and maintains effective therapeutic relationships with veterans and their families. </li><li>Works with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group counseling and therapy skills. </li></ul> <p>VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU</p> <p>Work Schedule: Monday through Friday8am-4:30pm</p> <p>Telework: Not Available</p> <p>Virtual: This is not a virtual position.</p> <p>Functional Statement #: 938360 (GS-9) 938370 (GS-11)</p> <p>Relocation/Recruitment Incentives:</p> <p>EDRP Authorized: This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program.
