<p>Job Summary:</p> <p>SUNY Upstate Cancer Center is seeking a highly skilled and motivated Advanced Practice Provider to join our Cancer Center as a Genetics APP to support the grow of the Genetic Cancer Program.</p> <p>Key Responsibilities:</p> <ul> <li>Assist with genetic testing on all patients newly diagnosed breast, ovarian, pancreatic, and colorectal cancer. </li><li>Seeing appropriate follow-up patients, screening for a genetic mutation that increases their risk of future malignancies. </li><li>Conduct collaborative, systematic, ongoing and criterion-based evaluation of the outcomes and continue to assess to revise the diagnosis, outcomes, plan, and implementation as needed. </li><li>Evaluates the accuracy of the diagnosis and the effectiveness of interventions and adapt the plan of care as appropriate for patients. </li><li>Communicates with leadership the patient, family, and system during all transitions of the patients care. </li></ul> <p>The APP is a member of the multidisciplinary Cancer Center Genetics Healthcare Team who demonstrates professionalism and leadership within the institution and exemplifies the Upstate Mission, Vision and Values and acts in accordance with Upstate policies and procedures.</p> <p>Minimum Qualifications:</p> <ul> <li>Master's Degree as a Nurse Practitioner or Physician Assistant (or graduated from an accredited PA program) or will be eligible by time of appointment. </li><li>NYS Licensed/Registered NP or PA. </li><li>Current National Certification consistent to the clinical practice. </li><li>Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire. </li><li>CPR-BLS Certification required at the time of appointment. </li><li>Must successfully pass pre-employment and credentialing clearances. </li></ul> <p>Preferred Qualifications:</p> <p>Prior experience in specialty area preferred.</p> <p>Experience in Oncology Genetic testing/counselling preferred.</p> <p>Work Days:</p> <p>Monday - Friday, Days</p> <p>Message to Applicants:</p> <p>In return for your dedication and commitment to provide outstanding care you'll receive:</p> <ul> <li>Generous paid time off program that includes vacation, holidays, and sick days. </li><li>Choice of several outstanding health insurance benefits and Well-being plans that include Student Loan Relief Services, Tuition Assistance, Adoption Assistance and an Employee Assistance Program. </li><li>Retirement options with employer contributions, plus additional voluntary retirement savings plans. </li></ul> <p>Recruitment Office: Human Resources</p>
