<p>Position Title: Health Aide - Supportive Housing</p> <p>Department: PACE</p> <p>Supervisor: Supportive Housing Manager</p> <p>Status/Hours Per Week: Non-Exempt / 40 hours</p> <p>Pay Range: The expected base pay for the position is $16-$28.05 per hour and may be increased based on other factors, such as language, certifications, etc.</p> <p>Primary Function:</p> <p>The Health Aide assists in the daily operation of the Adult Day Health Center and in the care of its participants. Duties include personal care, restorative therapy, nursing assistance, and recreational and escort responsibilities.</p> <p>The Health Aide is expected to be flexible and willing to assume clinical and some basic clerical and administrative tasks at the request of the ADHC Program Manager. The duties and responsibilities stated are subject to annual or semiannual competency evaluation. The expectation is for Health Aides to demonstrate competency performing the assigned tasks. The Health Aide draws upon all members of the IDT to enhance their skill set.</p> <p>Position is 40 hours per week: Monday - Thursday (7am-3pm), Saturday (11pm-7am), Friday and Sunday (off).</p> <p>Duties & Responsibilities:</p> <ul> <li>Demonstrates the ability to relate well to participants, anticipate their needs and encourage independence. </li><li>Assists participants with personal care. </li><li>Attends and actively participates in the daily and weekly clinical care team meetings. </li><li>Participates in the PACE QAPI Program; participates in the evaluation of Health Aide Services provided at PACE Centers, and in the implementation of QAPI interventions. </li><li>Assists with implementation of the care plan determined by the clinical team, and as directed and supervised by the Nurse Manager and/or ADHC RN and ADHC Manager. These responsibilities and duties may include but are not limited to ostomy care, skin care, incontinent care, simple dressings and wound care, vital signs, blood glucose tests, and weights. </li><li>Greets and assists with ADL set-up for participants upon arrival and departure from ADHC. </li><li>Carries out treatments under the direction and supervision of the Occupational and/or Physical Therapist including functional transfers, positioning and mobility. </li><li>Communicates participants' status to the clinical staff. </li><li>Assists with preparation of meals and feeding under direction and supervision of the Nutritionist and Occupational Therapist. </li><li>Assists with group activities, such as exercise, music, arts and crafts, special events. </li><li>Keeps food properly stored and assists with housekeeping duties in ADHC. </li><li>Assists with ordering supplies. </li><li>Attends appropriate in-services. </li><li>Other clinical and administrative tasks will be assigned as needed by the ADHC Program Manager. </li><li>May be assigned to be the lead on clinical and administrative tasks in collaboration with the ADHC Manager. </li></ul>
