This position provides assessments and therapeutic services to individuals, couples, families, and groups. This position works to provide mental health, addictions, and behavioral health clinical treatment/diagnosis within scope of practice as defined by degree and license. This position is responsible for participating in treatment planning with the center psychiatrist and client, reporting client progress to appropriate referral source, and maintain adequate and timely documentation. This position operates under minimal supervision and must exercise tact, judgment, and confidentiality in contact with the public, clients, and families, as well as fellow associates of the Center. Work is very broad in scope and requires a high level of professionalism.</p> <p>Position Responsibilities include but are not limited to:</p> <ul> <li>Provide individual, group, family therapeutic services, and case management as needed to clients. </li><li>To provide client contacts and meet assigned agency encounter expectation. </li><li>Meet with the client and their family (if appropriate) as outlined in the treatment plan. </li><li>Complete client assessments that include intake, evaluating a client's ability to meet the admission criteria for that particular department, identify presenting problem(s), history, medical information, family involvement, psychosocial/psychosexual information, and other pertinent information to form an appropriate clinical impression including substance use/abuse and abuse/neglect. </li><li>Complete a treatment plan, which accurately reflects the client's needs and ability to meet discharge criteria with goals and objectives that are time limited and behaviorally stated. </li><li>Ensure treatment plans receive all appropriate and necessary approvals and staffed in a timely manner. </li><li>Ensure treatment plans are updated prior to expiration. </li><li>Document timely, accurately and in accordance with Center policies utilizing current systems and practices. </li><li>Complete the case consult, transfer, and discharge summary processes according to agency policy and provide any referral source information according to the needs of the referral source. </li><li>Complies with the supervision policy specific to licensed professionals (LP) and licensing expectation policy specific to continuing education of the Center. </li><li>Completes all necessary and required monthly progress reports and ensures appropriate care coordination. </li><li>Document accurate information, which reflects client progress based on the appropriate therapeutic modality. Therapeutic services will be time limited and specific to the presenting problem(s) of each assigned client. </li><li>Provide progress reports to referral sources as needed for each individual client.
