Our dedicated team of 300+ staff members delivers integrated medical, dental, behavioral health, and support services to individuals and families most in need.</p> <p>We are currently seeking a compassionate and skilled Advanced Practice Provider (APP) - either a Nurse Practitioner or Physician Assistant - to join our Primary Care team.
