<p>Apply</p> <p>Job Type</p> <p>Full-time</p> <p>Description</p> <p>Care Coordinator- Children's Services</p> <p>People Incorporated Mental Health Services is seeking a Care coordinator to provide Care Coordination for our Youth Services Programs. This is an Intermediate level position that is responsible for working directly with consumers to understand their needs and goals. Collaborates with staff across the agency to ensure consumers are connected with the services they need. Also responsible for collaborating with external agency partners to coordinate care.</p> <p>Schedule: Full-Time Monday - Friday, 8:00am - 4:30pm OR 9:00am - 5:00pm. Hybrid work schedule available, with required days onsite.</p> <p>Location: New Hope Mental Health Clinic: 5555 Boone Ave. North, New Hope, MN 55428</p> <p>Hiring Range: $23.76 - $25.35 Hourly depending on qualifications and work experience, Plus $1,000 hiring bonus for external candidates</p> <p>People Incorporated Mental Health Services is the largest nonprofit mental health provider in Minnesota. For over 50 years, we have been a leading community partner, transforming the health of our communities through innovative solutions.</p> <p>Principal Accountabilities/Performance Standards:</p> <ul> <li>Assess consumer needs in the Preliminary Screening process, connect with services, and ensure continuity of care among internal providers and between internal and external providers. </li><li>Facilitate internal and external collaboration necessary to develop person-centered or family-centered integrated treatment plans based on consumer needs and recommendations of team members. </li><li>Interact with other team members to provide comprehensive and timely assistance in resolving issues that are barriers to the consumer receiving services, sharing information that enhances or improves access to services, and/or supports consumer engagement with services. </li><li>Conduct interview and gathers information to inform completion of intake paperwork, assessments, functional assessments, treatment planning, plan-of-care reviews, and discharge plans, as needed or directed. </li><li>Document and maintain accurate and timely case notes, family member(s) or identified supports approved by the consumer, and all activities tied to the consumer in the Electronic Health Record. </li><li>Provide direct assistance to consumer in making appointment and obtaining supports necessary to keep the appointment. </li><li>Provide various skill-building, teaching, and/or coaching, as it relates to the individual's goal and/or treatment plan. </li><li>Ensure that a crisis plan has been created by the appropriate staff, is maintained and followed. </li><li>For consumers who receive more than one CCBHC service, ensure that a care coordination agreement which outlines roles and responsibilities for supporting the consumer in accessing care/services specific to that consumer is completed. </li><li>For consumers who are receiving at least one CCBHC service and one other service in any People Incorporated program, the coordinator will review documentation of services received, provide a written proposal to providers identifying goal/objective/intervention areas where treatment plan integration could occur, and facilitate communication between providers within a schedule that meets the 90-day revised treatment plan timeline. </li><li>As allowed by data privacy rules, when the coordinator knows a consumer is seeking Hospital Emergency Dept. services, they will contact the ED social worker to provide their assessment of the situation, contact information for the coordinator and a current medication list. When possible, Coordinators will visit the individual in the hospital and will participate in hospital discharge planning meetings. </li><li>Develop working relationships with internal and external referral resources. </li><li>Monitor completion of Quality Measures and follow up as needed. </li><li>Provide general knowledge and guidance on assisting clients in the multiple phases of applying for and receiving services and benefits, as directed. </li></ul> <p>Requirements</p> <p>Required Qualifications:</p> <ul> <li>Master's degree in a behavioral science or related field; </li></ul> <p>OR</p> <ul> <li>Bachelor's degree in a behavioral science or related field and proof of a practicum or internship that requires direct interaction with adults or children served and is focused on behavioral sciences or related fields; </li></ul> <p>OR</p> <ul> <li>Has completed 30 semester credits or 45 quarter credits in behavioral science or a related field and: </li><li>Has proof of a practicum or internship that requires direct interaction with adults or children served and is focused on behavioral sciences or related fields; OR </li><li>Has 2,000 hours of supervised experience in the delivery of services to people with Mental Illness, DD, ID or TBI </li><li>Is fluent in the non-English language of the cultural group to which 50% of the practitioner's clients belong; </li></ul> <p>OR</p> <ul> <li>Has a high school diploma and 4,000 hours (two years) of experience in the delivery of services to people with </li><li>Mental illness; OR </li><li>Traumatic brain injury or developmental disabilities and completes training on mental illness, mental health de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effect </li></ul> <p>OR</p> <ul> <li>Has a high school diploma and 2,000 hours (one year) of experience in the delivery of services to people with: Mental illness, emotional disturbance, or substance use disorder, and receives clinical supervision from a mental health professional once a week until 4,000 hours of experience is met. </li></ul> <p>Preferred Qualifications:</p> <ul> <li>Master's degree in a behavioral science or related field </li><li>Bachelor's degree in a behavioral science or related field and proof of a practicum or internship that requires direct interaction with adults or children served and is focused on behavioral sciences or related fields </li><li>2,000 hours of supervised experience in the delivery of services to people with mental illness </li></ul> <p>Benefits:</p> <ul> <li>Accrue up to 4 weeks of PTO, plus company-designated holidays and floating holidays </li><li>403(b) Retirement Savings Plan with a 3% employer-match </li><li>Multiple health and dental insurance plan choices available </li><li>Lifestyle Benefit - choice between company contribution to health savings account, student loan repayment assistance, or flex time </li><li>Employer-paid Short & Long-Term Disability Insurance & Life Insurance </li><li>Free access to People Incorporated's Training Institute, offering free CEUs and a wide array of learning and education opportunities </li><li>Eligibility for state and federal loan forgiveness programs </li></ul> <p>We are committed to providing salary ranges for all open positions.
