<p>Employment Type:</p> <p>Full time</p> <p>Shift:</p> <p>Day Shift</p> <p>Description:</p> <p>Position Purpose:</p> <p>The Medical Group Lead Concierge provides patient care coordination by serving as a liaison between the patient and the private practitioners and services patients may require. Acts as a patient advocate, providing patient and family education, and coordinating all aspects of care to ensure the provision of seamless comprehensive services. The position plays a critical role in the patient experience including community outreach, optimizing patient services and working collaboratively with the team to create a patient centered environment. This involves developing expertise in the operations and systems of the enterprise related to promoting and maintaining health and wellbeing of the patients in our community. The scope of navigation extends across all service lines and specialties and is intended to coordinate care across the continuum in a comprehensive way. The concierge is an integral part of the organization's commitment to provide holistic patient centered care and improve patient experience. In addition to these responsibilities, the Lead Concierge leads and manages the workflow of a team of centralized schedulers, helping to ensure scheduling processes are efficient, accurate, and aligned with established guidelines.</p> <p>What you will do:</p> <ul> <li>Facilitates all aspects of care to provide seamless comprehensive services for patients based on their individual needs. </li><li>Maintains accurate data related to calls and referrals, both requested and scheduled. </li><li>Maintains up-to-date and accurate documentation of scheduling guidelines to ensure consistency and compliance across the team. </li><li>Leads and manages workflow of a team of centralized schedulers, providing guidance, training, and performance feedback to ensure efficient and high-quality scheduling operations. </li><li>Communicates with referring physicians' offices as required by patient and provider. </li><li>Serves as direct contact for Collins and other affiliate providers to facilitate the referral process for patients to a specialty provider. </li><li>Triages incoming calls and assist patients to schedule appointments via referrals process from providers. </li><li>Provides positive service excellence and educating patients and their families as needed. </li><li>Acts as an advocate/liaison for patients to ensure all support services are coordinated. </li><li>Provides interdepartmental support to patients who obtain services through the system. </li><li>Utilizes appropriate channels of communication to problem solve patient care issues. </li></ul> <p>Minimum Qualifications, Required Knowledge, Skills and Abilities:</p> <ul> <li>High School Diploma or equivalent is required; some college preferred. </li><li>Two to four (2-4) years of healthcare related experience is required. </li><li>This position requires a person with a positive attitude who is self-directed, pleasant, cooperative, assertive yet compassionate displaying a professional demeanor with patients, families, physicians and fellow workers. </li><li>Ability to provide guidance and support to team members. </li><li>Must be able to understand and relate concepts behind specific ideas and remember multiple tasks given over long periods of time. </li><li>Must be able to take initiative and work well independently with minimal supervision. </li><li>Must have exceptional social skills and emotional intelligence. </li></ul> <p>Position Highlights and Benefits:</p> <ul> <li>Full Time 40 hours M-F days </li><li>Our Mission and Core Values </li><li>Great benefits and Health Insurance Coverage-- even for Part Time! </li><li>Career growth and advancement potential </li><li>Award-winning Hospitals and Departments </li></ul> <p>We are a group of primary care providers, specialists and surgeons serving patients in Connecticut and western Massachusetts.
