<p>The Public Health Nurse primary responsibilities include providing prenatal, postpartum and parenting services to families facing varied, significant sources of stress. Public health nurses visit families in homes and the community. Bicultural and/or bilingual candidates with close ties to cultural communities such as (but not limited to) African American, East African, Hmong, and Latine, are encouraged to apply. Interventions provided are relationship-based and include education, case management, and support. Much of the work is done in the evidence-based MECSH (Maternal Early Childhood Sustained Home Visiting) model. This is not a home care nursing position. PHN will also provide general Public Health Nursing duties within the Public Health Department, including but not limited to emergency preparedness response.</p> <p> </p> <p>Washington County Public Health & Environment values your experiences, perspectives and unique identity. We are committed to building and sustaining an equitable and inclusive work environment and believe diversity benefits and enriches our programs, our staff and communities we serve.</p> <p> </p> <p>DESIRABLE EXPERIENCE:</p> <ul> <li>Bilingual in Spanish, proficient in verbal and written skills </li><li>Paid work experience with diverse cultures and interpreters and/or ability to serve families in a non-English language </li><li>Recent paid work experience with family health (maternity/prenatal, postpartum, pediatric clinic, and/or fatherhood engagement work) </li><li>Paid work experience with families who are at high risk for poor health outcomes (i.e. low socioeconomic, chemical health or mental health issues) </li><li>Paid work experience providing home visits to families </li><li>Recent paid public health experience </li><li>Previous paid home visiting experience in an evidence-based model (i.e. MECSH, Healthy Families America, Nurse-Family Partnership) </li><li>Paid work experience with electronic documentation in PHDoc, maintaining timely and accurate records </li><li>Advanced degree or certification related to maternal-child health </li></ul> <p>SELECTION TECHNIQUE: Your training and experience will be rated based on the required application material submitted and the top-scoring candidates will be forwarded to the hiring department for further consideration.</p> <p>HOURS / DAYS: 8:00 a.m. to 4:30 p.m. Monday-Friday with occasional evening or weekend commitments. This position may be located in any of our Service Centers based on programmatic need, with the potential for limited telework. Work schedules (hours and days) and location are subject to change based on department needs.</p> <p>ANTICIPATED HIRING RANGE: $34.94-$41.31 (This position may be hired at the Public Health Nurse II level depending on qualifications.)</p> <p>E-Verify Participation: Washington County participates in the federal E-Verify program. Employers may not use E-Verify to pre-screen job applicants and may not limit or influence the choice of documents you present for use on the Form I-9.</p> <p>E- Verify Participation Poster</p> <p>Right to work PosterThe Public Health Nurse I provides Public Health Nursing services and information related to family health, disease prevention, and control, health promotion and emergency preparedness to improve health outcomes for individuals, families, communities, and populations in need within the county.</p> <p>MINIMUM QUALIFICATIONS: This position requires a Bachelor's Degree in Nursing with an emphasis on public health or equivalent. Candidates must be a Registered Nurse within the State of MN and have certification as a Public Health Nurse in the State of Minnesota.</p> <p>CORE COMPETENCIES, JOB SPECIFIC KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED FOR SUCCESSFUL JOB PERFORMANCE </p> <p>Core Competencies outline essential business and professional traits necessary to perform this job.</p> <ul> <li> Promote a diverse, culturally competent, and respectful workplace; </li><li> Demonstrates working knowledge of public health principles, programs, and services; </li><li> Demonstrates knowledge of federal, state and local laws, rules, regulations, policies, and reimbursement mechanisms pertaining to public health services; </li><li> Demonstrates knowledge of counseling and education methods and concepts; </li><li> Demonstrates knowledge of community resources; </li><li> Demonstrates thorough knowledge of the application of nursing practices; </li><li> Demonstrates considerable knowledge of public health nursing theory; </li><li> Demonstrates the ability to apply principles of public health to communities, families, and individuals; </li><li> Demonstrates ability to provide skilled nursing care to clients; </li><li> Demonstrates ability to assess nursing needs and to develop and carry out nursing care plans; and </li><li> Demonstrates the ability to make nursing decisions within practice standards. </li></ul> <p>Provide Public Health Nursing services:</p> <ul> <li> Perform individual and family Public Health Nursing assessments evaluating mental, physical and social health status. </li><li> Assess the immunization status of children and adults; administer immunizations to children and adults. </li><li> Provide case management for new refugees or clients diagnosed with active or latent tuberculosis, or perinatal hepatitis B; Make referrals to appropriate agencies to meet client needs. </li><li> Assist with the investigation of infectious diseases reports </li><li> Refer families to community resources and make service recommendations; follow up on referrals and facilitate connections to resources </li><li> Consult and Collaborate with childcare providers, medical providers, school staff, human services, county staff, and other community partners on public health issues and concerns; </li><li> Participate in the execution of plans in response to a public health emergency. </li><li> Provide health education: Educate families on the maintenance of health and prevention of disease; </li><li> Instruct caregivers to recognize early signs of disease and deviations from typical health patterns; </li><li> Provide support to individuals, families, and groups related to a healthy pregnancy, positive parenting and child growth, and development. </li><li> Conduct training for employees and external partners on infectious disease prevention and control, immunizations and emergency preparedness; </li><li> Teach evidence-based principles of healthy living and apply information from related fields (nutrition, dental hygiene, safety, etc.); </li><li> Provide evidence-based home visiting services utilizing interventions and a standardized curriculum to support the relationship-based practice. </li></ul> <p> </p> <p>Provide program support:</p> <ul> <li> Maintain current case file/client documentation. </li><li> Collect and document outcome data for local, state and federal reporting. </li><li>Evaluate health trends and risk factors to help determine priorities for targeted interventions; </li></ul> <p>Serve as Public Health Department liaison by addressing various public and private organizations, community groups, and members of the general public concerning policies, programs, responsibilities and activities to inform and elicit community support.</p> <p>Perform other duties as assigned.</p> <p>WORK ENVIRONMENT </p> <p>Work performed is semi-sedentary in nature, and typically performed within the Government Center, a satellite office or other fieldwork location. Lifting requirement of up to 25 pounds on an occasional basis. Equipment used may include, but is not limited to, computer, calculator, telephone, camera, and copier/scanner/fax machine.</p> <p>Work interruptions are frequent. There is some pressure associated with serving individuals who may be emotionally charged and with project or activity deadlines and details. May involve exposure to environmental conditions, exposures may include, but not limited to, disagreeable odors, elevated noise levels, communicable diseases, body fluids and work with persons with behavioral and mental challenges.
