<p>Schedule: Work Saturday-Sunday from 9:30 AM to 6 PM, Monday-Wednesday from 8 AM to 4:30 PM, with Thursday and Friday off</p> <p>Work Model: Onsite</p> <p>About the role</p> <p>As a Patient Advocate you will make an impact by assisting patients to navigate the healthcare system. You will be a valued member of the Cognizant team and work collaboratively with patients, stakeholders and other teams.</p> <p>In this role, you will:</p> <ul> <li> <p>Assist patients' complete forms, applications, and all Medicaid related paperwork.</p> </li><li> <p>Collect and review patient information to determine patient's eligibility.</p> </li><li> <p>Prepare documents, review for accuracy and completeness.</p> </li><li> <p>Provide technical assistance on agency issues, services, and programs.</p> </li><li> <p>Maintain and/or create files or record keeping systems. Sort, label files, and retrieve documents or other materials.</p> </li><li> <p>Interact with patients at the site to provide real-time service support, address patient concerns.</p> </li><li> <p>Develop and retain professional relationships with hospital staff.</p> </li><li> <p>Handle caseload, retrieve, sustain, and communicate all designated reports.</p> </li><li> <p>Maintain confidentiality and adhere to EMTALA and HIPAA guidelines.</p> </li><li> <p>Other duties as assigned.</p> </li></ul> <p>At Cognizant, we strive to provide flexibility wherever possible, and we are here to support a healthy work-life balance though our various wellbeing programs. Rest assured; we will always be clear about role expectations.</p> <p>What you need to have to be considered</p> <ul> <li> <p>1+ year of work experience in health care, medical insurance, hospital, or clinic</p> </li><li> <p>High School or GED required; associate degree or equivalent from 2-year college a plus.</p> </li><li> <p>Bilingual Spanish required.</p> </li><li> <p>Comfortable working in a fast-paced diverse environment</p> </li><li> <p>Ability to work both independently and collaboratively within a team environment.</p> </li><li> <p>Excellent communication skills to interact with patients and hospital staff.</p> </li><li> <p>Ability to travel locally to other work sites.</p> </li><li> <p>Proficient in Microsoft Office (Word, Excel pivot tables, Project, and PowerPoint)</p> </li><li> <p>Familiarity with medical terminology and healthcare processes.</p> </li><li> <p>Ability to work weekends after training is complete/depends on individual learning process.
