In addition, the incumbent provides geriatric and/or palliative coordination and oversight for the CLC. The following duties are required of the position; they include, but are not limited it:</p> <ul> <li>Coordinates comprehensive, interdisciplinary, medical care and case management in CLC for Veterans with complex medical, social and behavioral conditions. </li><li>Participates in the establishment of policies, procedures, and guidelines necessary for clinical care in collaboration with other members of the interdisciplinary team. </li><li>Acts as a collaborating physician for APP providers in the CLC. </li><li>Monitors performance of medical care provided by Physicians and providers in the CLC. </li><li>Oversees admissions to the program. </li><li>Provides consultation to the medical staff for patients with complex treatment or care needs in the home. </li><li>Collaborates in the monitoring of quality and safety concerns within the program. </li><li>Communicates relevant responsibilities, policies and procedures to providers. </li><li>Responsible for maintaining medical standards of care, as mandated by accrediting bodies such as the Joint Commission. </li></ul> <p>Work Schedule: Monday - Friday, 8:00 am to 4:30 pm</p>
