<p>Employment Type:</p> <p>Full time</p> <p>Shift:</p> <p>Description:</p> <p>Mercy Home Health is seeking a Quality Management Specialist for the local Home Care branch office. Must live locally.</p> <p>Required Certification: OASIS Certification (COS-C or HCS-O) required within 6 months from date of hire. Must maintain recertification with no lapse in certification period.</p> <p>Mercy Home Health is seeking a Quality Management Specialist. This position requires a Therapy Clinical Licensure such as PT or OT.</p> <ul> <li>Required Certification: OASIS Certification (COS-C or HCS-O) required within 6 months from date of hire. Must maintain recertification with no lapse in certification period. </li></ul> <p>POSITION PURPOSE: Identifies specific performance improvement initiatives and works collaboratively with the local agencies/leaders in their development, implementation and evaluation of quality improvement programs. Performs identified workflow review and completion to achieve standardization and organizational efficiencies. Works collaboratively with agency, assuring compliance with new regulations, standards in EMR documentation.</p> <p>ESSENTIAL FUNCTIONS</p> <ul> <li>Identifies, collaboratively plans, implements and evaluates agency quality assurance/quality improvement/process improvement programs, focused on attainment of top decile outcomes, patient satisfaction and strategic aims. </li><li>Supports in-service education and other continuing education activities related to quality, outcomes, QA/PI. Monitors publicly reported outcomes and other data, utilizing quality software and EMR reports. </li><li>Assures agency staff compliance with federal/state and accreditation regulations through record review, case conferencing, mentoring and survey readiness/oversight functions. </li><li>Oversees Agency QA quarterly and annual reporting on QAPI, quality program initiatives. </li><li>Review discharge, death at home and other OASIS documentation for accuracy, completeness, and recommends corrections, utilizing appropriate software and OASIS guidance. </li><li>Collaboratively leads agency MDQI program, developing/ implementing QAPIs, action plans to attain top decile outcomes, patient satisfaction, strategic goals and regulatory compliance. </li></ul> <p>MINIMUM QUALIFICATIONS</p> <ul> <li>Therapist with current license from respective State Board. </li><li>Bachelor's Degree required. </li><li>Four years' experience in homecare. Minimum 2 years in a supervisory, oversight role preferred. </li><li>Certification: OASIS Certification (COS-C or HCS-O) required within 6 months from date of hire.
