<p>Position Summary</p> <p>The Registered Nurse Coordinator in this position is responsible for the care coordination of the Structural Heart Program as part of a team of nurses, cardiologists, and cardiac surgeons.</p> <p>Essential Duties and Responsibilities</p> <ul> <li>Receives and reviews patient referrals for appropriateness, reviewing with an attending provider as needed for any structural intervention. </li><li>Establishes contact with patient/family to explain process and work-up and testing schedule. </li><li>Facilitates obtaining all outside imaging, scheduling screening procedures and structural heart clinic visit and any additional testing. </li><li>Performs functional assessments and frailty testing for this patient population. </li><li>Prepares all testing available for review by cardiologist and cardiac surgical team at clinic visit. </li><li>Participates in development of treatment plan and performs patient and family teaching. </li><li>Facilitates direct contact between cardiologist and referring physician regarding treatment plan. </li><li>Uploads required data to appropriate vendor and maintain communication with device representatives. </li><li>Facilitates scheduling and authorization of structural heart procedures. </li><li>Contacts the patient 48-72 hours post discharge and then as needed until stable. </li><li>Participates follow-up visits and coordinates further follow-up per registry requirement. </li><li>Initiates and coordinates outreach programs. </li><li>Initiates and coordinates education for hospital staff </li><li>Tracks required registry data to be sure requirements are being met. </li><li>Performs other duties as assigned. </li><li>Work must be performed on site </li></ul> <p>Minimum Qualifications</p> <p>The requirements listed below are representative of the knowledge, skill, and/or ability required. Bachelor's Degree </li><li>Registered Nurse in DC </li></ul> <p>Experience</p> <ul> <li>Five (2) to seven (4) years as an RN Cardiac Experience or Cath lab experience </li><li>Sound knowledge of cardiovascular health and related diagnostic services </li><li>Clinical assessment, planning, intervention, and evaluation skills. Excellent communication skills </li><li>Strong ability to problem solve and multitask. </li></ul> <p>Physical Requirements</p> <ul> <li>Walk, bend, twist, and reach above the shoulder frequently in an office setting. </li><li>Must be able to occasionally lift or carry and frequently push or pull up to 100 lbs. as part of the role </li><li>Regularly exposed to healthcare settings that may require personal protective equipment </li><li>Requires manual dexterity to operate a computer keyboard, calculator, copier machine, and other equipment. </li></ul> <p>About GW MFA</p> <p>MFA physicians provide comprehensive patient care, offering one practice for the whole person with 52 medical and surgical specialties.
