<p>ESPERANZA HEALTH CENTER</p> <p>POSITION DESCRIPTION: Lifestyle Medicine Coordinator</p> <p>Department: Lifestyle Medicine</p> <p>Supervisor: Dr. Daniel Chen</p> <p>Status: Part-time, non-exempt (16 hours/week)</p> <p>Funding: 1-year grant-funded position</p> <p>Revised: 12/1/25</p> <p>POSITION SUMMARY</p> <p>Esperanza Health Center is seeking a highly organized, creative, and energetic individual to join our multi-award-winning team as the Lifestyle Medicine Coordinator. This role supports the growth and expansion of our Lifestyle Medicine programs, grounded in the six pillars of Lifestyle Medicine and in the mission to advance health equity, wellness access, and Kingdom-centered holistic health.</p> <p>The Coordinator will provide program administration, participant support, data collection, kitchen and food coordination, and Spanish translation of program materials. This role requires flexibility, strong communication skills, and the ability to manage multiple tasks in a fast-paced, community-centered environment. A reliable vehicle is required for weekly grocery pick-ups, deliveries, and travel between all three EHC sites.</p> <p>ESSENTIAL FUNCTIONS</p> <p>Program Administration & Documentation</p> <ul> <li> <p>Manage participant intake processes, including collection of waivers, questionnaires, surveys, and consent forms.</p> </li><li> <p>Support Lifestyle Medicine Assessments using standardized tools:</p> </li><li> <p>ACLM Diet Screener</p> </li><li> <p>Perceived Stress Scale</p> </li><li> <p>Sleep Quality Index</p> </li><li> <p>Assist with vital signs and laboratory measurement coordination (non-clinical; with clinical staff support where applicable).</p> </li><li> <p>Maintain organized digital and physical records for program tracking and reporting.</p> </li></ul> <p>Participant Coordination & Communication</p> <ul> <li> <p>Serve as primary liaison for Lifestyle Medicine participants, responding to questions, coordinating schedules, and providing follow-up communication.</p> </li><li> <p>Support participant recruitment, welcome/onboarding, and continued engagement throughout the program.</p> </li><li> <p>Coordinate with volunteers, students, Community Ambassadors (CAs), and Medical Assistants (MAs) to support program activities.</p> </li></ul> <p>Program Materials, Evaluation & Spanish Translation</p> <ul> <li> <p>Develop and distribute program handouts, recipes, and educational materials.</p> </li><li> <p>Translate slides, handouts, and recipes into Spanish to support bilingual program delivery.</p> </li><li> <p>Assist with program evaluation through data entry, participant feedback collection, and outcome tracking.</p> </li></ul> <p>Kitchen & Food Management</p> <ul> <li> <p>Oversee kitchen organization, including dishwashing, cleaning, inventory, and supply restocking.</p> </li><li> <p>Manage food ingredients and supplies for weekly sessions, food demonstrations, and group visits.</p> </li><li> <p>Coordinate weekly grocery pick-up, delivery, and storage from partners such as:</p> </li><li> <p>Sharing Excess</p> </li><li> <p>Get Fresh Daily</p> </li><li> <p>Philly Foodworks</p> </li><li> <p>Facilitate food access for non-SMA participants through partner vendor systems (e.g., Philly Foodworks credits).</p> </li></ul> <p>Logistics & Scheduling</p> <ul> <li> <p>Coordinate program calendars, session schedules, volunteer assignments, and participant reminders.</p> </li><li> <p>Ensure that all program spaces (kitchen and group areas) are organized, supplied, and prepared for sessions.</p> </li></ul> <p>QUALIFICATIONS</p> <p>Required</p> <ul> <li> <p>Passion for holistic health, wellness, and lifestyle medicine principles.</p> </li><li> <p>Strong organizational, administrative, and communication skills.</p> </li><li> <p>Creative, energetic, and able to work independently and collaboratively.</p> </li><li> <p>Reliable vehicle and valid driver's license (required for grocery pick-ups and inter-site travel).</p> </li><li> <p>Proficiency in Microsoft Office and/or Google Workspace.</p> </li><li> <p>Bilingual English/Spanish with strong translation skills (verbal and written).</p> </li></ul> <p>Preferred</p> <ul> <li> <p>Experience in community health, public health, nutrition, health coaching, program coordination, or related setting.</p> </li><li> <p>Familiarity with Lifestyle Medicine principles and assessments.</p> </li><li> <p>Experience working with volunteers, students, or community-based programs.</p> </li></ul> <p>CORE COMPETENCIES</p> <ul> <li> <p>Commitment to Esperanza's Christ-centered mission and values.</p> </li><li> <p>Cultural humility and ability to work effectively with diverse communities.</p> </li><li> <p>Strong interpersonal skills and a welcoming, participant-centered approach.</p> </li><li> <p>Ability to manage multiple priorities with attention to detail.</p> </li><li> <p>Creativity, initiative, and enthusiasm for expanding innovative wellness programs.</p> </li></ul> <p>WORKING CONDITIONS</p> <ul> <li> <p>Ability to stand, lift, carry, and transport groceries and supplies (up to 25 lbs).</p> </li><li> <p>Regular travel required between EHC sites and external food partners.</p> </li></ul> <p>Evening or weekend hours may be required occasionally for special events.</p>
