<p>Our Hospice team has an immediate opening for a PRN Admissions/Visits/Liaison Nurse to be part of our expansion to the northern Virginia area. Responsibilities:</p> <p>Perform initial assessment of patients' needs Obtain fully informed consents before admitting patients to the hospice program Serve as a clinical resource for referral sources to patients/family members Facilitate end of life conversations and decision-making with the family or caregiver. Prepare and discuss with physician an initial plan of care for patients Establish working relationships that promote utilization of JSSA Hospice by hospitals, physicians, community-based case managers and other community agencies. Represent our agency well when communicating with patients, families, physicians, and when participating in community network meetings. Flexible work schedule - must be available to take at least 2 shifts a month any time between Monday and Sunday as needed.</p> <p>Position Qualifications:</p> <p>An active RN license to practice in State of Virginia A minimum of 3 years of acute care and/or hospice experience. Demonstrated knowledge and skills necessary to provide meaningful care to and communicate with the terminally ill. Excellent communication and customer service skills; attention to detail; strong organization skills. Proficient with Microsoft Office 365 applications and the Homecare Homebase electronic medical record system.
