They fulfill patient care duties and administrative tasks, liaising between patients and physicians.</p> <ul> <li>Provides telephonic outreach to patients identified as needing follow-up from ordered referrals to specialty care and diagnostic testing. Patients with orders may have chronic health conditions and need preventative services. </li><li>Addresses and coordinates Transitions into Care after a Hospital discharge. </li><li>Assist in identifying gaps in care utilizing NextGen and Eagle Dream/Population Analytics. Care Gaps include cervical cancer screenings, colon cancer screenings, breast cancer screenings, well child checks, immunizations, and annual wellness exams. </li><li>Supports Care Management Care Plans through telephonic & electronic health record management, increasing health outcomes for patients </li></ul> <p>Who We Are:</p> <p>HealthSource of Ohio is a not-for-profit community health center in Southwestern Ohio that provides quality, patient-centered primary and preventative care for all.
