Once on site, this person collects information and registers patients for their appointment.</p> <ul> <li> <p>Operates hospital vehicle to transport Mammography Technologist and equipment to various sites in a safe manner, adhering to Mercy Health policies and DOT regulations.</p> </li><li> <p>Completes daily pre check and post check maintenance checklist to ensure van and equipment are in working order. Reports any malfunction of the vehicle to management.</p> </li><li> <p>Gases vehicle on specified days of the week, maintains cleanliness of van and stocks supplies when necessary.</p> </li><li> <p>Meets and greets customers. Obtains and verifies patient demographic information and ensures completion of forms to include: copies patient insurance card, Medicare questionnaire, obtain signed consent form and ABN when necessary</p> </li></ul> <p>MINIMUM EDUCATION</p> <p>Required Minimum Education: High School Diploma or GED</p> <p>LICENSURE/CERTIFICATIONS (must be non-expired/active unless otherwise stated):</p> <p>Required: Valid State Driver's License and CDL Class B.</p> <p>As a Bon Secours Mercy Health associate, you're part of a Mission that matters.
