<p>Job Description Summary</p> <p>We are seeking a dedicated and experienced Primary Care Physician (MD/DO) to join our outpatient clinic team. Patients trust us to receive the right personalized care where and when they need it - in our care centers, at home or virtually - to improve their health outcomes and quality of life.</p> <ul> <li> <p>Provide comprehensive, patient-centered primary care in an outpatient clinic setting.</p> </li><li> <p>Manage chronic and acute medical conditions with a focus on preventive care and quality outcomes.</p> </li><li> <p>Proactively address and document care related to HEDIS and STAR measures to improve health plan quality ratings.</p> </li><li> <p>Participate in and contribute to high-risk patient huddles and care coordination with clinical staff and support teams.</p> </li><li> <p>Collaborate with clinic leadership to monitor and improve clinic productivity, efficiency, and performance metrics.</p> </li><li> <p>Foster a positive patient experience through compassionate care, clear communication, and continuity.</p> </li><li> <p>Support Nurse Practitioners and Physician Assistants with clinical oversight, consultation, and mentorship as needed.</p> </li><li> <p>Attend staff and performance meetings to support operational goals and improve care delivery.</p> </li></ul> <p>Requirements:</p> <ul> <li> <p>Requires a MD or DO.</p> </li><li> <p>Current unrestricted medical licensure in applicable state(s) and board certification. If board eligible, must become board certified within 12 months.</p> </li><li> <p>DEA license required.</p> </li><li> <p>Satisfactory completion of a Tuberculosis test is a requirement for this position.</p> </li></ul> <p>Compensation: $232,000K - $282,785K & bonus eligible</p>
