Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services.</p> <p>Our community Health Care Centers are located at 281 Main St. East Hartford, 16 Coventry St. Hartford and 828 Sullivan Ave., South Windsor. In addition, under the direct supervision of the Program Manager or designee, he/ she coordinates and facilitates social programs and groups for residents.</p> <p>Essential Duties & Responsibilities:</p> <ul> <li>Responsible for supervision and safety of residents in all daily activities, including monitoring as required, room safety checks, </li></ul> <p>providing orientation and responding to all requests and emergencies.</p> <ul> <li>Maintains all necessary documentation on client activity during shift </li><li>Conducts individual and group social-rehabilitation and care coordination activities </li><li>May be required to transport clients in personal or Agency vehicle </li><li>In the event of staff shortage, may be required to work past scheduled hours until a relief staff can be found </li><li>Perform other related tasks as required </li></ul> <p>Schedule:</p> <p>Sunday - Thursday, 3 pm to 11:30 pm - Full-Time</p> <ul> <li>All agency staff are required to attend all mandatory department/agency meetings and trainings* </li></ul> <p>Requirements</p> <p>Education &/Or Experience:</p> <p>Associates Degree in Human Services field or high school diploma (G.E.D.) and a working knowledge of substance abuse and/or mental health treatment.</p> <p>Competencies:</p> <ul> <li>Initiative </li><li>Time Management </li><li>Decision Making </li><li>Communication Proficiency </li><li>Organization Skills </li><li>Computer competency </li></ul>
