<p>The Metro Nashville Public Health Department (MPHD) is seeking a nurse professional with the passion and experience required to identify and refer to care people infected with hepatitis C virus (HCV) or at risk for HCV infection.</p> <p>If you have any questions about this position please contact MPHD HR at (615)340-5640.</p> <ul> <li>Identifies and engages all individuals infected with hepatitis C virus (HCV) in Nashville-Davidson County </li><li>Oversees and ensures the investigation, interview, and case status determination for all suspected cases of acute HCV by utilization of clinician information, laboratory results, and/or Tennessee Department of Health (TDH) notification within 30 days </li><li>Provides test results and post-test counseling, determines risk factors, and elicits contacts of the individuals infected with HCV </li><li>Identifies, engages, and provides referrals to harm reduction services (PrEP, syringe service programs, and/or substance use treatment) for MPHD clients who are not hepatitis C infected but have risks factors for infection </li><li>Ensures demographics, contact attempts, referrals, and other pertinent information are entered into the electronic databases such as, Routine Electronic Data Capture (REDCap) and National Electronic Disease Surveillance System (NEDSS) based system (NBS), and other health records in a timely manner </li><li>Refers clients to HCV treatment and/or other services such as HIV care, mental health services, substance use disorder treatment based on client-centered goals. </li><li>Tracks and follows up on referrals to specialists </li><li>Communicates as necessary with other navigators in other jurisdictions </li><li>Collaborates with community-based organizations (CBOs) and other approved facilities to provide on-site HIV and HCV point of care testing or blood draws and referrals for linkage to care and other prevention services </li><li>Develops and maintains a region-specific provider directory detailing HCV, HIV, and mental health and substance abuse providers </li><li>Submits required reports such as the updated provider directory quarterly to TN Dept of Health </li><li>Maintains current knowledge of the Tennessee Department of Health Reportable Diseases </li><li>Maintains current knowledge of CDC/CSTE case definitions as it pertains to viral hepatitis </li><li>Prepares reports and presentations for audits, conferences, educational sessions, etc. </li><li>Participates in calls and meetings with project managers, central office HIV/HCV cluster response team, local epidemiologists, and/or clinic staff (as needed) </li><li>Assists in outreach events, screenings, and surge capacity as requested by Supervisor or higher authority </li><li>Licensed as a Registered Nurse in the State of Tennessee. Applicants holding a Registered Nurse license from a state that participates in the "Enhanced Nurse Licensure Compact" may be hired, but must obtain their Tennessee license within thirty (30) days of hire. </li><li>Diploma or Associate's Degree in nursing. Candidates with Diploma or Associate's Degree in nursing must establish a documented ability to competently perform all functions of this classification to be considered. </li><li>Cardiopulmonary Resuscitation (CPR) Certification. </li><li>Valid class "D" driver's license, use of personal vehicle, and maintenance of valid personal vehicle insurance as required by Tennessee Law. </li><li>All public health nurses are required to complete a department approved Certificate in Public Health as a condition of continued employment. Possess required competencies upon hiring. </li></ul> <p>Candidates with accreditations earned in a foreign institute are encouraged to apply, however at the present time MPHD is not sponsoring individuals through any of the US Government's employment-based immigration programs.</p> <p> </p> <p>MPHD seeks to be a socially and culturally responsive organization that acknowledges and addresses historical and current inequities in our public health promotion and practice.
