<p>Prototypes Pomona Residential Women's Center is one of HealthRIGHT 360's programs. Prototypes Women's Center is a 180-bed residential facility for women and women with children. This residential facility provides comprehensive substance use disorder (SUD), mental health, and psychiatric care by an interdisciplinary team of professionals. Treatment episode ranges in time from 60 days to 6 months.</p> <p>The Intake Coordinator I is responsible for the initial assessment of candidates to determine medical necessity, completion of program admission forms, ASAM assessment, and urgent referrals regarding medical and mental health needs. The Intake Coordinator I works in conjunction with the intake team to provide a warm hand off to the client's treatment team, assuring new clients are stabilized, and urgent needs have been addressed. The Intake Coordinator I will work closely with Counselors, Case Managers, Therapists, and other team members to coordinate and monitor client issues. In addition, Intake Coordinator I provides individual and group counseling, psychoeducation, relapse prevention, harm reduction, crisis intervention, and case management.</p> <p>This is a union position.</p> <p>KEY RESPONSIBILITIES</p> <p>Direct Service</p> <ul> <li>Conduct initial assessment and intake evaluation for the formulation of treatment plan. </li><li>Consult regularly with other staff to ensure clients meet criteria for safe admission to an appropriate level of treatment. </li><li>Provide case management services (e.g. housing, employment, finance management, court reports, DCFS communication). </li><li>Identify and implement relapse prevention or harm reduction interventions when indicated. </li><li>Provide group, family, and individual counseling. </li><li>Facilitate group sessions focused on substance use recovery, trauma recovery, co-occurring mental health symptoms, and other personal change topics. </li><li>Routinely review treatment plans. </li></ul> <p>Clinical Documentation</p> <ul> <li>Maintain compliance with the due dates for all clinical documentation. </li><li>Maintain confidential client records and prepare client progress reports and other required documentation in a timely manner. </li><li>Accurately enter data in various electronic systems for all clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements. </li></ul> <p>Administration and Other Duties</p> <ul> <li>Establish community relations and linkages with other service providers and community services. </li><li>Actively participate in clinical supervision, case conferences, and treatment team reviews of client progress. </li><li>Attend meetings and actively participate. </li><li>Complete assigned training on time. </li><li>Meet service delivery goals. </li><li>Read and respond to communications in a timely manner. </li><li>Maintain full compliance with registration or certification requirements. </li></ul> <p>QUALIFICATIONS</p> <p>Education, Certification, or Licensure</p> <ul> <li>High school diploma or equivalent required. Bachelor's degree in Health and Human Services, Social Work, Psychology, or other related field preferred. </li><li>Registration or Certification as a Substance Use Disorder Counselor from an accredited California agency (CCAPP, CAADE, CADTP). </li><li>Valid California Driver's License and access to registered and insured transportation. </li></ul> <p>Experience</p> <ul> <li>At least one year of experience working in the substance abuse treatment field. </li><li>Experience in culturally diverse substance abuse populations with co-occurring disorders or with history of incarceration desired. </li><li>Experience conducting assessments and providing appropriate diagnoses. </li><li>Experience with mental health Medi-Cal procedures. </li><li>Familiarity and/or experience with Drug Medi-Cal and ASAM Criteria preferred. </li></ul>
