The Case Manager works under the supervision of the YACAP Program Manager and in collaboration with the medical care team and other staff to ensure the goals and contractual obligations of the YACAP program are met, with a strong focus on community outreach and engagement of young adults and adolescents at increased risk for HIV.</p> <p>The Case Manager is responsible for timely data collection and entry into electronic medical record and reporting systems pursuant to contract needs and program requirements. This person will lead and assist in the implementation of community-based outreach activities, support recruitment and retention efforts, and help develop and implement quality improvement activities while representing the agency in the community. The focus of this role is HIV prevention education, PrEP/PEP navigation, HIV testing and counseling, and linkage to HIV care and treatment services for youth and young adults.</p> <p>DUTIES AND RESPONSIBILITIES:</p> <ul> <li>Provides intake, screening, and comprehensive psychosocial assessment of YACAP participants ages 13-29 to identify HIV prevention needs, risk factors, sexual health concerns, and barriers to accessing HIV testing and care. </li><li>Works closely under the supervision of the YACAP Program Manager and collaborates with medical providers, social workers, PrEP navigators, and other team members to coordinate comprehensive HIV prevention services, including PrEP (Pre-Exposure Prophylaxis) and PEP (Post-Exposure Prophylaxis) navigation, HIV testing, linkage to PrEP care, and linkage to primary and HIV specialty care as needed Assists with recruitment, enrollment, and retention of patients into programs. </li><li>Provides ongoing case management, including referrals and follow-up for medical services, behavioral health, mental health support, substance use services, social support, and prevention services, and documents clinical outcomes of all referrals. </li><li>Assists with recruitment, enrollment, and retention of participants ages 13-29 into the YACAP program through targeted outreach in clinics, community-based organizations, schools, colleges, youth centers, LGBTQ spaces, social media platforms, dating apps, and other community venues frequented by young people. </li><li>Ensures timely and accurate data collection and entry into eCW, AIRS, and/or other required electronic systems in accordance with funder requirements, program metrics, and agency standards. </li><li>Assists with developing, implementing, tracking, evaluating, and reporting program quality improvement projects related to HIV prevention outcomes, PrEP uptake and retention, linkage to care, and retention in services. </li><li>In collaboration with the Program Manager, helps ensure that any HIV-positive participant is promptly linked to HIV medical care within established timeframes, attends follow-up medical appointments, receives adherence counseling, and when applicable, has medical case management with initial assessment, re-assessment, and individualized care plan in accordance with Ryan White guidelines and agency policies </li><li>Participates in local, city, and other LGBTQ and youth-focused networks, coalitions, and task forces, including HIV prevention and linkage initiatives, as assigned by the Program Manager Participates in local, city and other HIV and sexual wellness related networks. </li><li>Complies with the agency mission, employee health policies, and ID and LGBTQ program protocols, including all confidentiality, documentation, mandatory reporting requirements for minors, and service delivery standards. </li><li>Performs other related duties as assigned by the manager. </li></ul> <p>EDUCATION & EXPERIENCE:</p> <ul> <li>High School Diploma or satisfactory equivalent required. </li><li>Associate degree from an accredited college or university in a related field is preferred. </li><li>Experience working with adolescents, young adults (ages 13-29) is strongly preferred. </li><li>Knowledge and experience working with people living with and affected by HIV/AIDS, including issues related to HIV prevention, PrEP/PEP, and/or treatment preferred. </li></ul> <p>#LI-DF1</p>
