<p>The Coordinator provides professional nurse management of Admissions and Medical Records and is responsible for admission process activities conforming with regulatory requirements in order to address community needs for placement to the facility to efficiently accomplish its mission. Position coordinates medical record functions within the facility to maintain accurate and orderly records meeting the regulatory requirements.</p> <p>Responsibilities:</p> <p>Medical Records</p> <ul> <li>Manage Medical Records functions including compile, verify, file, and ensure the medical record is properly processed and reviewed for completion. </li><li>Safeguard medical records and related information against loss, destruction or unauthorized use. </li><li>Audit departments to ensure compliance with state, federal and organizational regulations/requirements. </li><li>Reviews medical records for completeness, assembles records into standard order, and files records in designated areas according to applicable alphabetic or numeric filing systems. </li><li>Oversees the release of Medical Record process including timely response to legal correspondence, release of record, conferring with legal departments. </li><li>Oversees the release of Medical Records to other providers, regulatory agencies, billing agencies, and auditing agencies including Medicare, Medicaid, and third-party billers. </li><li>Scans, uploads, and files hospital records, labs, pathology reports, and loose correspondence into resident's electronic and paper records. Trains and assists unit secretaries with their records mgmt. duties. </li><li>Responds to and completes internal and external requests for records according to policies/regulations. </li><li>Coordinates process of purging and destruction of records for off-site records. </li><li>Performs other duties as assigned. </li></ul> <p>Admissions</p> <ul> <li>Completes review and assessments of admission referral documents and coordinates the patient admission functions in keeping with the mission and vision of the SNF. </li><li>Coordinate and facilitate the admission of patients from the referral sources by assessing referred documents and SNF capabilities. </li><li>Explain the pre-admission and admission policy to families and referral sources as necessary. </li><li>Conduct in-house tours for families and outside agencies as needed. </li><li>Collect and distribute admission information to all clinical services and the business office. Collect and coordinate clinical information responses to obtain payer authorizations. </li><li>Obtain Pre-Admission Authorizations and continued stay authorizations with Medicare, Medicare Advantage, Medicaid, and Insurance Programs. </li><li>Maintain access to Medicare common working files and calculate Medicare days. Access managed care plan data authorization systems to monitor approved stays. </li><li>Monitors SNF census and admission opportunities to obtain appropriate payer mix and SNF census according to budgetary plans. </li><li>Completes hospital and area agency networking visits to maximize referral sources and determine clinic referral needs. </li><li>Completes physical admission assessments and care planning upon request. </li><li>Completes medication administration pass upon request. </li><li>Notify referring facility of admission decision within established time frame. </li><li>Performs other duties as assigned. </li></ul> <p>Qualifications/Requirements:</p> <p>Experience:</p> <p>2-5 Years of experience in Healthcare Facility with strong knowledge of nursing medical terminology. Significant organizational skills and ability to multi-task. Knowledge of care and placement of individuals in Acute, Sub-Acute, Nursing Home, Assisted Living, and Home Care Programs. Obtains Patient Review Instrument Certification and Pre-Admission Mental Health Screening capability while on job.</p> <p>Education:</p> <p>Graduate from an accredited school of nursing.</p> <p>Licenses / Certifications:</p> <p>Must possess a valid license and current registration, issued by the New York State Education Department, as a Registered Nurse. Current AHA BLS required.</p> <p>Other:</p> <p>The individual performing this job may reasonably anticipate coming into contact with human blood and other potentially infectious materials. Individuals in this position are required to exercise universal precautions, use of personal protective equipment and devices, and learn the policies concerning infection control.
