Harbor Care is seeking a compassionate and skilled Per Diem Pharmacist to join our mission-driven team in Nashua, New Hampshire.</p> <p>At Harbor Care, pharmacy is more than dispensing medications-it's an integral part of whole-person care. You will collaborate closely with providers, support vulnerable populations including Veterans and individuals experiencing homelessness, and help ensure patients receive safe, effective, and accessible care.</p> <p>This role offers flexibility while giving you the opportunity to practice at the top of your license - providing clinical services, supporting care teams, and building trusted relationships with patients.</p> <p>What You'll Do:</p> <ul> <li>Accurately review, verify, and dispense prescriptions in compliance with state and federal regulations </li><li>Provide clinical services including immunizations and Medication Therapy Management (MTM) </li><li>Collaborate with providers to support optimal patient outcomes </li><li>Supervise pharmacy support staff and ensure smooth daily operations </li><li>Deliver exceptional, patient-centered service and counseling </li><li>Support compliance with HIPAA and all regulatory standards </li></ul> <p>At Harbor Care, you'll be part of an innovative organization integrating healthcare, housing, and supportive services to create real pathways out of homelessness.</p> <p>Required Qualifications:</p> <ul> <li>Active Pharmacist license in good standing with the New Hampshire Board of Pharmacy </li><li>Immunization certification from an accredited organization </li></ul> <p>Preferred Qualifications:</p> <ul> <li>Experience in a community pharmacy setting </li><li>MTM certification and experience </li><li>Familiarity with 340B programs </li><li>Ability to work a flexible, as-needed schedule </li></ul> <p>Knowledge, Skills, and Abilities:</p> <ul> <li>Strong clinical judgment and attention to detail </li><li>Ability to work independently and as part of a team </li><li>Excellent time management and workflow prioritization </li><li>Strong communication and customer service skills </li><li>Ability to manage confidential information and maintain HIPAA compliance </li></ul>
