<p>Social Worker - Center for Senior Wellness</p> <p>Full-Time | Huntington, WV</p> <p>LGSW, LCSW, or LICSW Required</p> <p>Valley Health Systems is seeking a passionate and dedicated Social Worker to join our Center for Senior Wellness. Enjoy a great schedule plus a comprehensive benefits package designed to support your wellbeing.</p> <p>What You'll Do</p> <ul> <li>Develop and lead our Dementia Support Program, including patient enrollment, caregiver education, behavior-management strategies, and resource coordination. </li><li>Host a Monthly Dementia Support Group and a Monthly Parkinson's Support Group, creating safe and supportive spaces for patients and caregivers. </li><li>Provide individual and group caregiver support sessions to help families navigate challenges confidently. </li><li>Conduct Advance Care Planning (ACP) sessions for patients and families. </li><li>Address Social Determinants of Health, such as transportation barriers, caregiver shortages, guardianship concerns, and nursing home or assisted-living placement. </li><li>Support patients and families through end-of-life decision-making and hospice referrals with empathy and clarity. </li><li>Collaborate closely with an interdisciplinary team to ensure holistic, patient-centered care. </li></ul> <p>What You Bring</p> <ul> <li>Master's Degree in Social Work. </li><li>Active WV LGSW, LCSW, or LICSW. </li><li>Supervision offered to LGSW to get fully licensed. </li><li>Experience with geriatrics, dementia, Parkinson's, or caregiver support preferred. </li><li>Excellent communication skills, emotional intelligence, and a passion for serving seniors. </li></ul> <p>Why You'll Love Working Here</p> <ul> <li>Great Schedule: Monday-Friday, daytime hours-with 2 evenings per month, but no nights, weekends, or holidays. </li><li>Comprehensive Benefits: Health, dental, vision, retirement plans, time off, professional development opportunities, and more. </li><li>Opportunity to build meaningful programs that directly support seniors and caregivers. </li><li>Mission-driven, collaborative team environment where your contributions truly matter. </li></ul>
