<p>Southwest Community Health Center</p> <p>Job Title: Behavioral Health Therapist</p> <p>Summary: Functions as part of primary health care team providing crisis intervention, assessment, interventions, brief treatment, long term therapy and referrals as appropriate for clients with behavioral health problems. Provides consultation to PCP's on behavioral health needs of clients.</p> <p>SPECIFIC DUTIES AND RESPONSIBILITIES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA).</p> <ul> <li>Provides psychosocial assessment, intervention, treatment and referral as appropriate to patients in need of these services, including children, adolescents and adults. </li><li>Intervene with clients identified by PCP to assess behavioral health needs. </li><li>Provide consultation to PCP on behavioral health matters as requested by PCP. </li><li>Provides immediate intervention for clients in crisis (e.g. expressing suicidal, homicidal ideation). </li><li>Link clients in need of Psychiatric assessment and/or medication to SWCHC Psychiatric providers and coordinate care. </li><li>Maintain client records and provide other documentation as required in accordance with agency, Joint Commission, DSS, DPH and other applicable standards. </li><li>Performs all other duties as assigned. </li></ul> <p>Qualifications:</p> <ul> <li>Excellent organizational, communication and interpersonal skills. </li><li>Computer literate. Capable of using Electronic Health Record for documentation. </li><li>Able to work collaboratively with primary care staff, Psychiatric providers and other Behavioral Health Clinicians. </li><li>Able to function efficiently in team based, integrated care setting. </li><li>CPR/BLS Certified </li></ul> <p>Education and/or Experience:</p> <ul> <li>Master's degree in Social Work, Counseling or Human Services. CT License required. </li><li>LCSW, LPC, LMFT preferred. </li><li>Three years' experience working in behavioral health field with knowledge of co-occurring issues and psychopharmacology. </li><li>Experience in providing services to medically underserved and culturally diverse populations. </li></ul> <p>Physical Requirements and Work Environment:</p> <p>The physical requirements and work environment described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. This position requires the manual dexterity sufficient to operate phones, computers and other office equipment. The position requires the physical ability to kneel, bend and perform light lifting. This person must have the ability to write and speak clearly using the English language to convey information and be able to hear at normal speaking levels both in person and over the telephone. Specific vision abilities required by this job include close vision, depth perception and the ability to adjust focus. Generally, the working conditions are good with little or no exposure to extremes in health, safety hazards and/or hazardous materials. This person must have the ability to travel as required to work with staff.</p>
