For more information, refer to Required Documents below.</p> <p>This Licensed Practical Nurse position is Nursing Services at the Boston VA Healthcare Systems, located at the Jamaica Plain location.</p> <p>The incumbent works under the direction of the Nurse Manager, delivering practical nursing care to patients with a variety of physical needs. Documents treatments and patient utilizing Computerized Patient Record System/Globally Unique Identifier (CPRS/GUI). The incumbent works under the direction of a Registered Nurse, maintaining a safe dignified environment.</p> <p>Duties may include, but are not limited to:</p> <ul> <li>Conducts assessments of medication. </li><li>Reports side effects and other symptoms to providers. </li><li>Conducts repeated medication reviews. </li><li>Provides ongoing education to patients about medications and other health-related behaviors by participating in group or individual meetings. </li><li>Assists patients develop plans to address inconsistencies in medication use. </li><li>Works with staff to deliver group and/or individual educational interventions to promote healthy behaviors. </li><li>Serves as the liaison to hospital and community providers. </li><li>Establishes relationships with family members and provide education for families as needed. </li><li>Supports the team with case management needs of the Veterans and assists the teams in encouraging Veterans to follow through with treatment plans. </li></ul> <p>Work Schedule: Monday thru Friday 8:00 am - 4:30 pm or 8:30 am - 5:00 pm.</p> <p>Compressed/Flexible:Not Available.</p> <p>Recruitment Incentive (Sign-on Bonus): Not Authorized.</p> <p>Permanent Change of Station (Relocation Assistance): Not Authorized</p> <p>EDRP Authorized: Former EDRP participants ineligible to apply for incentive. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).</p> <p>Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
