<p>Position Summary</p> <p>The Case Manager will provide targeted support to students navigating physical and mental health services, both on and off campus. This role involves care coordination, referral management, follow-up support, and collaboration with internal and external providers to ensure continuity of care. The Case Manager will be a key liaison between students and the appropriate campus services (academic advising, disability services, financial aid, etc.) and community resources that will support their success. This is a part-time (20-25 hours/week) position for 9 months of the year, when classes are in session.</p> <p>Key Responsibilities</p> <ul> <li> <p>Conduct intake assessments and develop individualized care coordination plans.</p> </li><li> <p>Assist students in accessing off-campus providers for specialized care (e.g., psychiatry, eating disorders, substance use treatment).</p> </li><li> <p>Track referrals and follow-up to ensure continuity of care.</p> </li><li> <p>Collaborate with staff to coordinate referrals to appropriate campus departments (e.g., counseling, health services, financial aid, academic advising, disability services).</p> </li><li> <p>Maintain accurate documentation of sensitive and private communications.</p> </li><li> <p>Provide psychoeducation and resource navigation support to students.</p> </li><li> <p>Participate in interdisciplinary team meetings and contribute to holistic care planning.</p> </li><li> <p>Assist with crisis intervention and safety planning as needed.</p> </li><li> <p>Primary contact for CARE referrals and link to appropriate resources as deemed appropriate by Early Intervention Planning team.</p> </li><li> <p>Maintain up-to-date knowledge of local health and mental health resources.</p> </li><li> <p>Serve as a standing member of the Behavioral Concerns Team and the Early Intervention Planning group.</p> </li></ul>
