<p>GRADE: 9 - Exempt REPORTS TO: Nurse Practitioner</p> <p>POSITION REQUIREMENTS:</p> <ul> <li>Graduate of an accredited school of nursing and licensed by the Virginia Board of Nursing as a licensed practical nurse. </li><li>License and current registration to practice as a licensed practical nurse in Virginia. </li><li>Minimum of one year's experience working with a frail or elderly population. </li><li>Compliance with any State or Federal requirements for direct patient care staff in their respective settings. </li><li>Valid driver's license. </li><li>Pass a criminal background check. </li><li>Medically cleared of communicable diseases and up to date with immunizations. </li><li>Must demonstrate ability to meet performance criteria for the position and possess knowledge and skills necessary to treat geriatric patients. </li></ul> <p>SUMMARY: Responsible for assessment of health needs and contributing to the plan of care, implementation of nursing care plans and providing nursing care, as well as periodic evaluation of participant nursing needs. Perform duties in clinic setting and home environments. Provide nursing care to participants under the direct and indirect supervision of the nurse practitioner, physician assistant, registered nurse and providers of primary care.</p> <p>DUTIES AND RESPONSIBILITIES:</p> <ul> <li>Perform and document nursing assessments for participants. </li><li>Provide treatments, medication administration, diagnostic and preventive measures and other services within the scope of a professional licensed practical nurse. </li><li>Provide routine and episodic nursing care according to the plan of care. </li><li>Complete immunization screening and ensure vaccinations are up to date. </li><li>Teach participants and caregivers regarding self-help in recognition and solution of physical, emotional, and environmental health problems. </li><li>Conduct periodic and unscheduled assessments of each participant according to the plan of care developed by the team. </li><li>Act as a coordinator of patient care with other health care providers. </li><li>Ensure timely, complete communication of referral services into the medical record. </li><li>Complies with HIPAA requirements and maintains strict confidentiality in all matters pertaining to PACE participants and program operations. </li><li>Participate in on-call coverage to troubleshoot and coordinate the scheduling of participants' care. </li><li>Comply with Mandated Reporter requirements reporting any suspected abuse, neglect or exploitation to the appropriate authorities. </li><li>Assist with staff development and training. </li><li>Participates in the Quality Improvement program. </li><li>Collect and analyze data. </li><li>Order, organize, and utilize supplies and equipment economically. </li><li>Perform other duties as assigned by the Registered Nurse, Nurse Practitioner, Physician Assistant, Medical Director or Program Director. </li></ul> <p>KNOWLEDGE, SKILLS AND ABILITIES:</p> <ul> <li>Critical thinking skills to identify problems requiring further referral and/or follow-up. </li><li>Skill in the application of current nursing procedures and techniques of patient care. </li><li>Ability to plan and coordinate nursing care for individuals, families, and groups. </li><li>Ability to identify and understand the medical and social needs of the frail elderly. </li><li>Knowledge of health care delivery systems and regulations. </li><li>Skill in interviewing and assessment techniques. </li><li>Able to function well in a team environment. </li><li>Possess good organizational, communication, and supervisory skills. </li><li>Ability to work effectively with socially, economically and educationally diverse population. </li><li>Able to effectively handle difficult and unusual interpersonal situations. </li></ul> <p>Offering competitive pay, generous leave policy and a family friendly work environment.</p> <p>15 paid holidays, Medical, Dental, and Vision coverage, Retirement, Short- and Long-term disability benefits, Critical Illness, Accident and Life Insurance.</p>
