<p>Requisition #:207594</p> <p>Location:Minneapolis, MN 55439</p> <p>Category:Administrative/Clerical</p> <p>Salary:$22.00 - $24.00 per hour</p> <p>Position Details</p> <p>Position Overview</p> <p>The Intake Coordinator is responsible for the coordination of new patient referrals, working in collaboration with Account Executives/Liaisons, referral sources, discharge planners, and location staff to ensure a seamless transition to agency services in compliance with Aveanna Healthcare policies, procedures, and federal/state regulations and guidelines.</p> <p>Schedule: Monday - Friday 8am - 5pm CST, with on call after hours and weekend opportunities</p> <p>Essential Job Functions</p> <ul> <li>Responds to referral source and office inquires by communicating accurate and complete information </li><li>Coordinates multiple telephone calls from internal/external customers and directs callers or tasks appropriately </li><li>Establishes good working relationships between the location and referral sources to ensure further referrals from that source </li><li>Reviews documentation sent by the referral source for accuracy and completeness </li><li>Documents all forms required to process a referral in its entirety with no missing information </li><li>Enters all referral information in real time into the Electronic Medical Record (EMR) to provide visibility across the team </li><li>Partners with colleagues in the location to staff each case with an appropriate, skilled professional to provide the services requested and ordered </li><li>Photocopy, fax, route, scan, or email information as needed or requested </li><li>Maintains department logs/tracking systems as requested </li><li>Other duties as assigned </li></ul> <p>Benefits Offerings:</p> <ul> <li>401(k) with company match </li><li>Health, dental, vision, life, and pet insurance </li><li>Mileage reimbursement and cell phone allowance </li><li>Generous PTO, sick time, and paid holidays </li><li>Inclusion Day to celebrate what matters to you </li><li>Float Day for extra flexibility and balance </li><li>Up to 8 Hours of Paid Volunteer time yearly </li><li>No-Cost Employee Assistance Program (EAP) - unlimited mental health telephonic counseling sessions, support with identity left, Will preparation and travel assistance </li><li>Robust DEI company program because Inclusion is an Aveanna Core Value </li><li>Tuition discounts and reimbursement </li></ul> <p>Requirements</p> <ul> <li>High school diploma or GED </li><li>Proficient in Microsoft Office, including Outlook and TEAMS </li><li>Ability to work after hours, on-call schedule </li></ul> <p>Preferences</p> <ul> <li>One (1) year previous office work experience preferred </li><li>Healthcare experience a plus </li><li>Bilingual experience </li></ul> <p>As an employer accepting Medicare and Medicaid funds, employees must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons as appropriate.</p>
