Ensures that students are either set up with a viable plan of support in their current school or, if not, actively supporting them through a transition to a school program which provides the academic supports needed for student success. Click here for more information on compensation for these roles. </li><li>In addition to competitive compensation, DPS has other Total Reward offerings such as; time off, health and wellness benefits, and PERA Retirement. </li></ul> <p>What You'll Do:</p> <ul> <li> <p>Responsible for the case management of students who are struggling in their current schools as they transition to a new educational setting (mostly students for whom other school-based supports have not worked). This includes maintaining consistent communication with school staff, establishing communication and relationships with students/families, providing school tours, supporting enrollment processes at new schools, and conducting follow-up meetings with students once they have transitioned.</p> </li><li> <p>Communicates with school staff, students and families to distribute information about school options, enrollment processes, and available pathways to graduation.</p> </li><li> <p>Supports the enrollment processes at Pathways School which are designed to support students who are off track academically reach graduation through accelerated academics and a range of additional supports.</p> </li><li> <p>Consults school-based administrators on possible interventions for students based on segment data and helps school leadership, counselors, and other school staff members at 3-5 designated high schools schools develop processes and practices for identifying struggling students, supporting transitioning students coming into their building. They facilitate training and support for school counselors, staff, and administration to build capacity for this work at the school level, and having a solid understanding of the school options, settings and resources available for students across the district.</p> </li><li> <p>Collaborates with 4-6 designated middle schools to support Transitions Day preparation, and to identify 8th graders who could benefit from starting high school at a Pathways school. They will then engage in case management with identified 8th grade students/families to explore Pathways school options. The Transitions Specialist will conduct check-ins at the start of the subsequent school year with the identified students who choose to enroll at a traditional high school.</p> </li><li> <p>Maintains data and documentation daily about relevant student information and all student/family interactions through the Infinite Campus and Salesforce databases.
