Healthy location. In this role, you'll support high-risk patients by helping them overcome barriers to care, access essential services, and improve their overall well-being.</p> <p>Key Responsibilities:</p> <ul> <li>Identify patients' needs and barriers to reaching their health and wellness goals </li><li>Work closely with Primary Care and Behavioral Health teams to develop care plans to ensure patients are receiving the care and resources they need. </li><li>Help patients set goals, solve problems, and navigate healthcare systems </li><li>Connect patients to critical services like mental health support, housing, and food assistance </li><li>Track progress, coordinate follow-ups, and advocate for patients at every step </li><li>Build strong partnerships with community organizations to expand support options </li></ul> <p>Qualifications:</p> <ul> <li>Bachelor's degree in social work, Psychology, Sociology, or a related field </li><li>Experience with both adult and pediatric populations </li><li>Knowledge of local resources and social support systems </li><li>Strong communication, time management, and organizational skills </li><li>Digitally proficient: Microsoft Office and EMR systems </li><li>Preferred: </li><li>1+ year of case management or community health experience </li><li>Bilingual (Spanish/English) </li></ul> <p>About Us:</p> <p>The HealthCare Connection (THCC) is a non-profit Federally Qualified Health Center (FQHC) serving the underserved, underinsured, and uninsured residents of northern Hamilton County since 1967.
