<p>Assists the department head in implementing, organizing and evaluating all services of the Home Care Program. Ensures services are rendered according to the plan of care and in accordance with policies and procedures within the Home Care Department. Supervises team to ensure work flows and processes are adhered to. Supports mission to provide the highest quality care by complying with regulatory guidelines, meeting facility standards and obtaining appropriate certification.</p> <p>JOB DUTIES / RESPONSIBILITIES:</p> <ol> <li> <p>Participates in the ongoing evaluation of all field clinicians within their designated team and their provision of care to ensure compliance with Home Care Program policies.</p> </li><li> <p>Conducts employee performance evaluations through daily performance, clinical record review and outcome of field evaluations.</p> </li><li> <p>Coordinates/communicates with interdisciplinary team to ensure continuity of patient care. Participates in System-Wide quality improvement program. Supervises the implementation of departmental quality activities.</p> </li><li> <p>Assists in preparation for D.O.H. accreditation and program certification. Involves staff at all levels in survey compliance.</p> </li><li> <p>Recommends and implements systems for documentation necessary to maintain accountability and compliance with external regulations and internal standards, and standardization within SMHCS.</p> </li><li> <p>Enforces the operations structure for the department and recommends work systems to ensure efficiency of personnel and resources.</p> </li><li> <p>Makes home visits to assess, evaluate and provide direct care responsibilities, as indicated or needed.</p> </li><li> <p>Provides feedback to staff when opportunities for improvement/excellence are observed. Performs coaching/disciplinary action as necessary.</p> </li><li> <p>Participates in recruitment and retention efforts.</p> </li><li> <p>Maintains budget within established parameters. Provides input in preparation.</p> </li><li> <p>Promotes customer friendly service. Investigates and resolves all customer complaints. Ensures customer satisfaction through quality improvement initiatives.</p> </li><li> <p>Submits written reports within established time frames.</p> </li><li> <p>Provides new & ongoing staff training and education. Assist in orientation.</p> </li><li> <p>Monitors daily admissions, discharges, hospitalizations, incidents and complaints and reports outcomes.</p> </li><li> <p>Responsible to ensure that all clinical staff are meeting productivity and monitors their schedules.</p> </li><li> <p>Reviews and tracks timely submission of all necessary documentation, MD orders, SOC's and therapy evaluations are made according to department policy and regulation.</p> </li><li> <p>Supervises all assigned clinical and office staff. Communicates as appropriate with staff, physicians, referral sources and clients. Coordinates daily workflow activities of assigned cases.</p> </li><li> <p>Performs ongoing review of cases and ensures appropriate delegation of assignments. Collaborates with schedulers so that all visits are conducted according to the physician ordered plan of care.</p> </li><li> <p>Serves as a liaison between professional staff and the Managed Care Department to ensure timely receipt of requested information.</p> </li><li> <p>Collaborates closely with all departments to ensure appropriate reimbursement for home care services.</p> </li><li> <p>Achieves proficiency in the computer program EMR and acts as a resource for all staff and field clinicians.</p> </li><li> <p>Ensures confidentiality and policies regarding HIPAA guidelines are met.</p> </li><li> <p>Develops and maintains good working relationships internally and externally to ensure ongoing referrals.</p> </li><li> <p>Ensures compliance with State and Federal regulations. Assists Department heads with preparing for a successful program survey.</p> </li></ol> <p>Formal education required:</p> <p>Associate's Degree in Nursing with 3 years of Nursing experience or Bachelor's Degree in Nursing, Physical, Occupational or Speech required; Master's Degree preferred.</p> <p>Currently licensed as an RN, PT, OT or ST in the State of New York.</p> <p>Previous experience required:</p> <p>Minimum of one year pediatric experience. Home Care experience preferred.</p> <p>Minimum of 2 years managerial and/or supervisory experience preferred.</p>
