<p>Company:</p> <p>Providence at Home with Compassus</p> <p>Territory: Orange County</p> <p>Schedule: Monday-Friday, 8am-5pm</p> <p>Position Summary</p> <p>Facilitation, coordination and communication between patient and family with referral sources to maximize patient's functioning capacity in the community. Continually exercises independent judgment and discretion, and within agreed upon limitations, makes patient management decisions involving interventions based on knowledge of the organization, policies, procedures, and professional code of ethics. Performs under minimal supervision.</p> <p>Position Specific Responsibilities</p> <ul> <li>• Assess patients' psychosocial, financial, environmental, and community needs, as documented in the initial social work assessment, Plan of Care (POC), clinical records, and team meeting notes. </li><li>Provide direct and indirect social services support for patients and families in all service settings (homes, long-term care facilities, hospitals). </li><li>Deliver crisis intervention and supportive counseling for patients and families. </li><li>Complete all documentation in compliance with federal/state hospice regulations and policies to ensure regulatory compliance. </li><li>Actively participate as a member of the interdisciplinary team, providing input to support patient/family care objectives. </li><li>Attend team meetings, contribute to care planning, and collaborate with colleagues to coordinate patient/family services. </li><li>Provide consultation and assistance to colleagues, administration, patients/families, and community partners as needed. </li><li>Support level-of-care changes and discharge planning for patients and families. </li><li>Respond promptly to referrals from the interdisciplinary team and community for social services needs. </li><li>Accept assignments consistent with education, experience, and competence. </li><li>Promote teamwork, effectiveness, and efficiency through a cooperative and professional attitude. </li></ul> <p>Education and/or Experience</p> <ul> <li>Required - Master's Degree from an accredited School of Social Work. </li><li>Required Experience: </li><li>Minimum of 1 year delivering social work services in a healthcare setting. </li><li>At least 3 years of experience in inpatient, outpatient, or home-based social work. </li><li>Independent and collaborative work within an interdisciplinary team. </li></ul> <p>Preferred Experience:</p> <ul> <li>Knowledge of research techniques, data collection, and policy/program/curriculum development. </li><li>Experience with quality improvement methodology. </li><li>Post-degree experience in a hospital or other healthcare setting. </li></ul> <p>Certifications, Licenses, and Registrations</p> <ul> <li>Required (Home Health Only): BLS (American Heart Association, Vendor Managed). </li><li>Preferred upon Hire: Social Worker License. </li></ul> <p>State Specific Requirements</p> <p>California</p> <ul> <li>Preferred: Social Worker License upon hire. </li></ul> <p>Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Routine use of standard medical equipment and tools associated with clinical care is essential.
