<p>Summary</p> <p>Under the general direction of the Medical Director and the Practice Manager, the Palliative Care Nurse Coordinator will work closely with the palliative care providers, other providers, care managers and nurses to coordinate comprehensive palliative care services. The palliative care nurse coordinator acts as a resource and support to patients, families and staff for palliative care and end of life issues.</p> <p>Certification, Registration, and Licensure Required:</p> <p>Current NH RN license; Certification in Palliative Care (CHPN) within one year of hire.</p> <p>Qualifications and Competencies:</p> <ul> <li>Strong organizational, communication, and interpersonal skills. </li><li>Knowledge of palliative care principles, end-of-life issues, and healthcare systems preferred. </li><li>Proficiency in electronic health records and Microsoft Office Suite. </li><li>Demonstrates effective time management skills and attention to detail. </li><li>Demonstrated an ability to work in a collaborative manner. </li></ul> <p>Experience:</p> <p>Minimum of 5 years experience in acute care.</p> <p>Responsibilities</p> <ul> <li>Represent and introduce palliative care services to patients, families, and healthcare providers. </li><li>Utilize the nursing process to assess and facilitate care related to the specialty needs of individual patients and families for successful palliative care management and intervention. </li><li>Review daily census, gather background information from referring providers, and medical records to triage consult requests and assist with determining priority. </li><li>Coordinate team consult schedules and manage incoming consult requests for both inpatient and outpatient settings. </li><li>Act as a resource on palliative care and end-of-life issues for patients, families, and staff. </li><li>Attend unit rounds when possible. </li><li>Coordinate with chaplain, social work, care management, and hospice/home care providers (GVNA) to ensure comprehensive patient support. </li><li>Provide follow-up calls and bedside check-ins as requested by Palliative Care providers. </li><li>Organize and assist hospitalists/intensivists in conducting family meetings in the absence of a Palliative Care Provider. </li><li>Collaborates with staff in other CHMG Palliative Care practice. </li><li>Perform data collection and data validation. </li><li>Actively participate in the Cancer Committee and other relevant committees to advocate for palliative care interests and initiatives within the organization. </li><li>Performs job-specific requirements and other duties as assigned </li></ul> <p>Concord Hospital is an Equal Employment Opportunity employer. Specific vision abilities required by this job include color vision, depth perception, far vision, near vision, and peripheral vision.</p> <p>The employee is frequently exposed to airborne pathogens, bloodborne pathogens, bodily fluids. The employee is occasionally exposed to airborne contaminants, chemotherapeutic agents, electrical hazards - shock, moving mechanical parts, non-weather-related heat or cold, slippery surfaces, toxic or caustic chemicals.</p> <p>The noise level in the work environment is usually quiet.</p>
