<p>JOB DESCRIPTION AND RESPONSIBILITIES:</p> <ul> <li>Responsible for performing a variety of casework duties, including meeting with residents, and preparing psychosocial evaluations, including obtaining, analyzing, and evaluating data and developing services plans. </li><li>Responsible for making the residents aware of the facility's rules and regulations and providing information about the service plan process. </li><li>Responsible for caseload and performs record keeping functions as required, case notes, census statistical reports while making sure all records are maintained at a confidential level. </li><li>Refers residents to agencies and programs as necessary and establishes a rapport and professional relationship with all stakeholders. </li><li>Assists residents in establishing relationships with outside services. Arranges for transportation to necessary appointments. </li><li>Prepares a facility workshop and meeting with clients relative to goals of Social Services and needs of residents and works closely with other personnel assigned to residents. </li><li>Handles emergencies as they arise arrange for medical services. </li></ul> <p>Attend regular staff meetings and documents progress to supervisor.</p> <ul> <li>Demonstrates the ability to recognize priorities in organization of workflow. Able to perform duties independently, with minimal need of direct supervision. </li><li>Shows the ability to interact positively and helpfully with clients, visitors, contract staff, volunteers, and co-workers. </li><li>Reflects commitment to building a supportive work environment and maintains a positive attitude at the workplace and towards job. </li><li>Projects a good image in dealing with the public and clients and is willing to make an extra effort to help build a quality and caring shelter/program. </li><li>Other tasks as assigned. </li></ul> <p>REQUIREMENTS:</p> <ul> <li>Bachelor's Degree in related field preferred </li><li>Knowledge of HIPAA (Health Insurance Portability and Accountability) </li><li>Staff scheduling experience </li><li>Problem solving skills, attention to detail, & organization skills </li><li>Proficiency in Microsoft Office </li><li>Excellent verbal and written communication skills </li></ul>
