Aspire for Well-Being Home Care is seeking compassionate and dedicated individuals to join our team as Home Care Assistants. In this rewarding role, you'll provide essential support to clients in their homes-ranging from companionship and personal care to specialized services and transportation-tailored to each individual's needs and service plan. If you have a heart for service, strong judgment, and a desire to bring positivity into someone's daily life, we'd love to meet you!</p> <p>The Home Care Assistant provides residents/clients and families with the highest quality of home care services which includes companionship, personal care, transportation, and home support as well as a wide variety of ancillary services depending on the specific needs and service plan of the client. Delivers Specialized Care following individualized training and competency criteria, in accordance with the Pennsylvania Department of Health guidance. All services listed in the service plan are provided, including any additional services requested if allowable by state regulations and Aspire for Well-Being Home Care (Aspire Home Care) policies. Services are provided in a caring and compassionate manner that recognizes individual needs, preferences, and rights.</p> <p>The content of this job description is intended to describe the general nature and level of work expectations performed by incumbents in this classification.</p> <p>Essential Functions:</p> <ul> <li>Provides companionship and home support service to promote resident/client independence, including but not limited to light housekeeping, laundry, linen changes, meal preparation, errands and grocery shopping, medication, and appointment </li><li>Assists resident/client with activities of daily living including but not limited to support with bathing, dressing, toileting, grooming, oral hygiene, skin care and /or ambulation and </li><li>Following individualized training and competency, provides specialized care to residents, defined in the Aspire for Well-Being Home Care policy. </li><li>Provides transportation as listed in the service plan and/or at the direction of the resident/client if services are allowed by state regulation and Aspire for Well-Being Home Care policies and procedures. </li><li>Observes and reports to immediate supervisor any observed changes in the resident/client's physical condition, mental / emotional state, or home situation in a timely </li><li>Demonstrates an understanding of time management principles by seeking to remain engaged and involved throughout the shift and providing "value-added" support when service plan tasks are </li><li>Demonstrates the ability to work independently and use good judgement. </li><li>Reads resident/client's previous shift notes and general service notes upon arrival and documents specific services provided when shift is completed. </li><li>Provides updates to supervisor at least weekly and provides resident/client information as requested by supervisor. </li><li>Coordinates specific work tasks with other personnel within the home care department as well as with other programs and departments to ensure the seamless quality provision of service and support to residents / clients. </li><li>Obtains clear signatures on paperwork as </li><li>Relates in a courteous manner to residents/clients, families, visitors, and fellow staff members and promotes respectful relationships in all interactions with others. </li><li>Protects the privacy, confidentiality and dignity of all residents/clients and co-workers. </li><li>Follows all policies and procedures and suggests methods for </li><li>Follows safe practices to ensure the health and well-being of residents/clients, families, and employees including self, including universal and standard </li><li>Maintains awareness of individual responsibilities regarding fire, health, and safety </li><li>Carries out work tasks in a manner that maintains health and wellness and reduces injuries and uses equipment and supplies safely and properly. </li><li>Completes skills assessment prior to providing care and annually </li><li>Communicates business-related issues in a positive manner that respects residents/clients, families and employees while maintaining professional boundaries. </li><li>Uses telephone or home care software system to log in and out of all direct care shifts and to provide documentation for services provided; Uses companywide Human Resource Management System (HRMS) for all work </li><li>Completes honestly and accurately all forms as required including but not limited to mileage, travel time, tasks completed, and telephonic time </li><li>Attends and or completes all mandatory orientation, in-service training and education programs including competency assessments and TB education at hire and annually thereafter and demonstrates an interest in improving the quality of services </li><li>Accepts supervision with a positive attitude and follows the chain of </li><li>An RN/LPN working under the Home Care license may provide non-skilled services within the scope of his/her nursing license. </li><li>An RN/LPN working under the Home Care license may provide applicable training/ competencies to Home Care Assistants. </li></ul> <p>Position Type/Expected Hours of Work:</p> <ul> <li>This is an as needed/per diem position with no guarantee of hours on a regular basis. </li><li>The position requires the ability to work during non-standard hours, including evenings, weekends, holidays, and during inclement weather, and to be available when the need </li></ul> <p>Travel:</p> <ul> <li>Travel may be expected for this position, including local travel, as required by a client service plan or service address. </li></ul> <p>Education and Experience Requirements:</p> <ul> <li>High School Diploma or GED preferred </li><li>1-3 Years' personal care/home care experience </li><li>May be a certified Nursing Assistant; however, not required for this position. </li></ul>
