<p>POSITION SUMMARY:</p> <p>The RN Care Manager - Obstetric (OB) Nurse Navigator serves as a nursing champion within Family Health Center's (FHC) interdisciplinary Maternal Care Clinic (MCC). In collaboration with the WMed Family Medicine Residency, the MCC provides comprehensive care to a diverse patient population throughout the prenatal, delivery, and postpartum periods.</p> <p>This role utilizes evidence-based clinical guidelines and protocols to create and implement individualized, patient-centered care plans. The RN Care Manager - OB Nurse Navigator supports clinical operations through expert nursing assessment and interventions, acts as a consistent resource for MCC patients, collaborates with providers to triage clinical concerns, and develops strategies to enhance community engagement.</p> <p>DUTIES AND RESPONSIBILITIES:</p> <p>Aligned with FHC's North Star objectives of Quality, Finance, Patient Satisfaction, Employee Engagement, and Growth, key responsibilities include, but are not limited to:</p> <ul> <li>Ensure high-quality care by adhering to best practice guidelines, tracking health outcomes, and complying with standards set by the state board of nursing, the State Nurse Practice Act, and other regulatory bodies. </li><li>Collaborate with the interdisciplinary care team to define care objectives and assess patient and family healthcare, education, and psychosocial needs. </li><li>Develop and regularly update individualized care plans; monitor patient outcomes and promote self-management tools for sustainable well-being. </li><li>Conduct RN-led OB intake visits, coordinate hospital transitions of care outreach, and provide post-delivery support and education. </li><li>Monitor and analyze trends within the MCC patient panel to support population health strategies. </li><li>Participate in provider-led visits and carry out delegated tasks such as performing nonstress tests (NSTs). </li><li>Ensure continuity and connection with pediatric care for newborn post-delivery. </li><li>Manage patient outreach, triage urgent concerns, and coordinate with medical, behavioral health, and community providers. </li><li>Oversee the FHC Free Pregnancy Clinic and initiate referrals and follow-up care as appropriate. </li><li>Initiate, lead, and participate in quality improvement projects to enhance maternal and infant health outcomes. </li><li>Represent FHC in relevant OB nursing workgroups, taskforces, and consortiums at local, regional, and national levels. </li><li>Maintain clear, objective documentation and ensure accurate billing/coding for services provided. </li><li>Participate in FHC and community events to foster strong organizational and community relationships. </li><li>Stay current in clinical knowledge and skills through ongoing education and engagement in professional development opportunities. </li><li>Comply with HIPAA and maintain strict confidentiality of patient information. </li></ul> <p>COMPETENCIES:</p> <p>Commitment to Patients</p> <ul> <li>Demonstrates expertise in obstetric nursing and leads initiatives aligned with strategic goals. </li><li>Communicates respectfully and professionally in all patient interactions. </li><li>Effectively prioritizes tasks to ensure safe and timely care. </li><li>Adheres to the nursing process and evidence-based clinical practices. </li></ul> <p>Commitment to Team</p> <ul> <li>Embraces change, seeks input, and collaborates to achieve shared goals. </li><li>Recognizes and supports others' contributions. </li><li>Provides constructive feedback, encourages positive team morale, and shares knowledge. </li><li>Actively contributes to meetings and fosters a collaborative team environment. </li><li>Plans, executes, and communicates projects effectively, managing resources and timelines. </li></ul> <p>Commitment to Professionalism</p> <ul> <li>Makes informed decisions, delegates appropriately, and constructively addresses conflict. </li><li>Communicates clearly in both verbal and written formats. </li><li>Demonstrates ethical behavior, integrity, and respect for all. </li><li>Exhibits creativity, accuracy, and thoroughness. </li></ul> <p>Commitment to the Organization</p> <ul> <li>Aligns work with organizational goals and completes tasks efficiently. </li><li>Demonstrates initiative in improving systems and processes. </li><li>Maintains a focus on safety, security, and compliance with data privacy standards. </li><li>Uses electronic medical records (EMR) and technology responsibly and effectively. </li></ul> <p>TYPICAL WORKING CONDITIONS:</p> <ul> <li>Moderate to quiet noise environment is typical. </li><li>Variable work hours may be required to meet organizational needs. </li><li>Exposure to airborne particles may occur. </li></ul> <p>TYPICAL PHYSICAL DEMANDS:</p> <ul> <li>Regular use of hands; frequent talking and hearing is required. </li><li>Frequent standing and sitting; occasional walking, reaching, and stooping. </li><li>Occasional lifting of up to 25 pounds. </li><li>Visual requirements include close vision, color vision, and the ability to adjust focus. </li></ul> <p>QUALIFICATIONS:</p> <ul> <li>Minimum 1-2 years of RN experience. </li><li>Experience with OB care including prenatal visits and fetal surveillance (NSTs). </li><li>Proficiency with electronic health record systems. </li><li>Reliable transportation to access various FHC locations and community partner sites. </li><li>Strong communication skills to interact effectively with patients and partners. </li></ul> <p>EDUCATION/EXPERIENCE/CERTIFICATIONS/LICENSES:</p> <ul> <li>Required: </li><li>Graduation from an accredited school of nursing. </li><li>Current active, unrestricted Michigan RN license. </li><li>Basic Life Support (BLS) certification. </li><li>Bachelor of Science in Nursing (BSN) or related field. </li><li>Three or more years of obstetric experience. </li><li>Relevant specialty nursing certifications. </li><li>Experience in community and/or public health settings. </li></ul> <p>Family Health Center has the right to modify the duties and functions of the job description based on the needs of the organization.</p>
