These values - integrity, patient-centered, respect, accountability, and compassion - must guide what we do, as individuals and professionals, every day.</p> <p>The Yale Pediatric Cardiology and the Yale Adult Congenital Heart Disease Program are seeking a qualified Genetic Counselor interested in working in a growing program that includes physicians specializing in the genetic basis of congenital heart disease, pediatric cardiomyopathy, inherited arrhythmias, and aortopathies. The mission of the program is to provide outstanding integrated patient care while keeping the academic commitment to teaching and performing research in different aspects of congenital heart disease. This position is critical to providing continuity of care to patients and families with inherited forms of heart disease. Clinical work will span the ambulatory clinics and inpatient units.</p> <p>EEO/AA/Disability/Veteran</p> <p>Responsibilities</p> <ul> <li> <ol> <li>Perform clinical daily duties: 1. Obtain and assess patient and family medical and psychosocial histories 2. Provide patient education, psychosocial counseling, coordinate testing, facilitate decision -making, and make referrals as necessary. 3. Discuss results of diagnostic tests with program team to determine direction of counseling plans. 4. Participate in weekly multidisciplinary case conference. </li></ol> </li><li> <ol start="2"> <li>Collaborate and Coordinate Services 5. Collaborate with the Congenital Cardiac Genetics Physician Leader to coordinate services. 6. Collaborate with the congenital cardiology inpatient team to provide consultative care for inpatients with congenital heart disease. 7. Develop written reports on personal and family medical histories and test results. 8. Serve as a Lead Genetic Counselor for evolving aortopathy program. </li></ol> </li><li> <ol start="3"> <li>Participate in education and quality-based programming 9. Participate in institution-wide educational activities as appropriate to role. 10. Identify opportunities for involvement in research, teaching, and community outreach. 11. Travel to outreach clinics as appropriate to support program. 12. Work closely with the congenital cardiac genetics research team to foster optimal patient participation in applicable research trials. 13. Assist in the development of a Quality Assurance Performance Improvement forum through which performance opportunities can be identified. </li></ol> </li></ul> <p>Qualifications</p> <p>EDUCATION</p> <p>Master's Degree in Genetic Counseling or Human Genetics from an ACGC Accredited Program.</p> <p>EXPERIENCE</p> <p>One year of experience as a genetic counselor preferred. Research experience is a plus.</p> <p>LICENSURE</p> <p>Board eligibility by the American Board of Genetic Counseling (ABGC) or American Board of Medical Genetics (ABMG) required; certification preferred. Eligible for licensure in the State of Connecticut.</p> <p>SPECIAL SKILLS</p> <p>Excellent written and verbal communication skills. Able to work in a team focused approach. Able to work autonomously.
