<p>Description</p> <p>Summary:</p> <p>Assists with the implementation of physical therapy services that identifies, prevents, remediates and rehabilitates acute or prolonged physical dysfunction or pain in the patient's home. Under the supervision of a physical therapist, delivers treatment including therapeutic exercise, physical modalities, patient and family education and procedures. Work is performed in the patient's place of residence.</p> <p>Responsibilities:</p> <ol> <li> <p>Works under the direction and supervision of the licensed Physical Therapist in the home environment practice setting.</p> </li><li> <p>Performs Physical Therapy procedures ad related tasks under the direction of the supervising Physical Therapist.</p> </li><li> <p>Provides patient/family education and training.</p> </li><li> <p>Performs routine operational functions, including superision of Home Health Aide.</p> </li><li> <p>Documents visits and completes documentation within 24 hours of visit and within agency guidelines.</p> </li><li> <p>May modify specific treatment procedure in accordancewith patient status within the scope of the established treatment plan.</p> </li><li> <p>Participates in Performance Improvement activities and peer review.</p> </li><li> <p>Completes continuing education as per agency and state licensing requirements.</p> </li><li> <p>Fulfills all compliance responsibilities related to the position.</p> </li><li> <p>Performs other duties as assigned.</p> </li></ol> <p>Education: ASSOCIATE'S LVL DGRE</p> <p>Other Information:</p> <p>Required: Customer relation skills with patients and staff.
