The nephrology team consists of 2 RN, 1 MA, and 3 APC's working with 10 MD's. This position will be responsible for delivering professional nursing care to patients via phone, health portal, Tele Health and Tele Home, and in outpatient settings. Document pertinent information in patients chart. </li><li>Performs clinical triage and provides telephone advice by assessing patient symptoms, determining urgency of care, and initiating appropriate interventions in collaboration with providers and the central triage team. </li><li>Discusses patient and family concerns over the phone and/or via the health portal secure messaging. </li><li>Provides and develops patient/family education materials surrounding common nephrology disorders. </li><li>Communicates adjustments in treatment plans, including medication adjustments, to patients and families. </li><li>Evaluates patient response to interventions and modifies plan of care as needed to achieve health care goals in conjunction with patient's providers. </li><li>Monitors, coordinates, and manages care for patients with chronic nephrology conditions, including ongoing assessment, care planning, and follow-up to support long-term health outcomes. </li><li>Develops and participates in educating patients and their families about care programs. </li><li>Communicates lab & other test results to families. </li><li>Assist with medication refills and arrangements for medication infusions. </li><li>Assists providers and staff with patient-related paperwork such as letters of medical necessity, DMV forms, school care plans, and pre-authorizations. </li><li>Maintains a safe and therapeutic patient care environment. </li><li>Works closely with central triage team to provide high quality, efficient care to patients and their families. </li><li>Keeps current on advances in nursing care by participating in educational programs. </li><li>Orients and educates new staff in division and departmental procedures. </li><li>Utilizes knowledge of medications, procedures, infection control, and safety precaution, and initiates appropriate interventions in crisis situations. </li><li>Participates in total quality management to positively affect patient and system outcomes to improve care. </li><li>Works with other parts of Clinical Enterprise (Triage, Scheduling, Referrals) to optimize patient care. </li><li>Assists in clinic operations ad hoc as needed, including supporting providers prior to and after patient visits. </li></ul> <p>Incumbent will be expected to provide high quality patient care in an organization focused on patient safety, patient and parent satisfaction, and positive patient outcomes; advocates for patients and families to ensure unique needs are met; collaborates with highly skilled team members dedicated to achieving patient goals and providing world class care; actively participates in educational opportunities. Education in patient management is ongoing with the help of division members.</p> <p>Minimum Qualifications</p> <p>Required</p> <p>Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire.</p> <p>Current license to practice as a Registered Nurse in the State of Utah, or obtain one within 90 days of hire under the interstate compact if switching residency to State of Utah. Must maintain current Interstate Compact (multi-state) license if residency is not being changed to Utah.</p> <p>Preferred</p> <p>Some positions may require additional certifications such as Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), or Neonatal Resuscitation Program (NRP) within one year of hire.</p> <p>This position is patient-sensitive and must fulfill all associated requirements.
