<p>Job Summary: Client Support Specialist use their personal experiences to assist team members in engaging and supporting people living with HIV over 50 and to ensure they are linked to the services they need.</p> <p>Major Activities:</p> <ul> <li>Incorporate best practices regarding confidentiality into all job duties and communications in accordance with HIPAA, Article 27-F, Alliance for Positive Health policies and procedures and other applicable regulations. Observe and abide by the HIV Confidentiality Law. </li><li>As applicable and within program guidelines, use personal disclosure and relating of life experiences to provide support to clients, encourage client engagement in services, and de-stigmatize treatment, testing and/or identified service needs. </li><li>Assist with targeted outreach to increase engagement of designated target populations. </li><li>Assist clients in self-advocacy and self-management skill-building. </li><li>Conduct follow up to service plan activities including appointment reminders and appointment accompaniment. </li><li>Help facilitate referrals to care management and other services as needed. Conduct follow-up to referrals as assigned. </li><li>Assist with facilitating psychosocial support groups. </li><li>Rotate through all regional offices to serve clients, conduct psychosocial support groups, and conduct outreach. </li><li>Participate as a member of the service team; communicate and coordinate with team members regarding assigned clients. Participate in regular client case review and case conferences as applicable. </li><li>Collect, maintain, and report up to date client health indicator, contact, and history data. Utilize indicator data to address barriers and improve individual health outcomes. Implement strategies to communicate client health information among internal and external interdisciplinary teams. Complete accurate, timely and thorough documentation in accordance with funder and agency requirements. </li></ul> <p>Qualifications:</p> <ul> <li>High school diploma or GED </li><li>Must have life experiences reflective of those faced by the target population </li><li>Familiarity with the region's services and navigating the systems of care and support </li><li>Must have a valid, unrestricted driver's license and have a vehicle to travel to designated appointment sites within a multi-county region. </li></ul>
