The ideal candidate will have excellent communication and organizational skills, with a proactive approach to promoting patient safety and continuous improvement.</p> <p>Duties include:</p> <ul> <li>Conduct and oversee all aspects of investigations as per Office of Mental Health (OMH) policies and procedures, including but not limited to: </li></ul> <p>o Interview staff and gather relevant documentation.</p> <p>o Coordinate Justice Center interviews with facility leadership for patients and staff.</p> <p>o Complete comprehensive reports on significant incident investigations within 30 days.</p> <p>o Identify incident causes, patterns, and trends to inform risk reduction strategies.</p> <ul> <li>Conduct risk management orientation and related training sessions for staff. </li><li>Participate in environmental rounds to identify and mitigate potential safety risks. </li><li>Review incident and restraint/seclusion forms, ensuring accurate entry into the NIMRS database. </li><li>Serve as the liaison between the facility, OMH field office, Central Office, and the NYS Justice Center. </li><li>On-Call Availability: Be available during off-shifts and weekends to address reportable incidents as needed. </li></ul> <p>Minimum Qualifications Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement - Statewide (NY HELPS) program if they have a bachelor's degree in a human service field and 2 year of social work experience.</p> <p>o Substitution: Master's degree in social work can substitute for 1 year of specialized experience.</p> <p>OR</p> <p>Candidates within State Service can be considered for a competitive class appointment if they are reachable on the current Social Work Assistant 3 eligible list.</p> <p>OR</p> <p>Candidates from within State Service can be considered transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.</p> <p>OR</p> <p>In accordance with Section 52.7 of the Civil Service Law, this notice advises all employees of the Office of Mental Health of our intention to conduct a non-competitive promotion examination for the position of Social Work Assistant 2 at Kingsboro Psychiatric Center.
